On 01/30/2024, Centralized Application Bureau (CAB) Licensing Program Analyst (LPA) Arianna Manabat and Licensing Program Analyst (LPA) Matthew Gallo met with the Licensee Representative Amy Warren and Director Andorra Fierro for the purpose of an announced pre-licensing inspection. The Applicant requests an infant component which will serve 8 children ages 0-2 years of age, a toddler component which will serve 12 children ages 18-36 months, a preschool component which will serve 30 children ages 2 years to entry into first grade. Today's census included 0 infant children, 0 toddler children, and 0 preschool children, the Director, and Licensee Representative.

The program will operate Monday through Sunday from 8:30 a.m to 4:00 p.m. "Before and After Care" is from 7:30-8:30am and 4:00pm-5:30pm. The Licensee Representative stated that the Saturday and Sunday opening is subject to the winter season as they are a part of the North Star ski resort. The fire clearance was received and granted on December 12th, 2023 for the Infant Component and January 18th, 2024 for the preschool and toddler component. The facility will be providing lunch and morning and afternoon snacks for all components. Food is prepared in the kitchen by staff. Parents will provide formula, breast milk, and baby food for the infant component.

LPAs reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted. Applicant acknowledges that the following documents must be posted at all times: License, Emergency Disaster Plan, Personal Rights, Parents' Rights Poster, Effects of Lead Exposure, car seat poster, menus, and daily schedule. Applicant stated that the facility does not provide snacks or lunch.

LPAs discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment

INDOOR ACTIVITY SPACE:
There is one room for the infants (room #2), one room for toddlers (room #3), and two rooms for the preschool space (rooms #1 and #4). Both LPAs observed sufficient amount of age-appropriate equipment, toys, tables, and cubbies within each classroom. For the infant classroom, there are five cribs available for use. The changing table and portable sink is inside of the crib space for the infant room. LPA advised the facility representative that the changing table may cause children in the crib space to wake up if the infants, being changed, are making audible noise. The facility representative stated that, if this does occur, the portable sink and crib will be relocated in the room and that the sink will remain in arms reach of a sink.

There is a first aid kit in each classroom inside each of the backpacks. Each first aid is equipped with sufficient amount of items including band aids, gauze, ice packs, and more. Medications will be stored in the office area. CAB LPA observed cleaning disinfectants are appropriately stored and inaccessible to children in a closet that is adjacent to the kitchen. This closet is locked. Applicant stated there are no poisons or firearms on the premises. Applicant stated that access to drinking water will be via water bottles for all components and that the water will be refilled in the kitchen area. In the infant room, the classroom is equipped with bottle warmers. LPAs observed and tested the carbon monoxide detector within the facility. LPAs observed a sign-in/sign-out sheet posted at the front entry point.

CAB LPA measured all classrooms. The facility's classrooms measured as a total of:
- 365.75 square feet for the infant classroom (room #2), 445.52 square feet for the toddler classroom (room #3), 1,438.74 square feet for the preschool classrooms (rooms #1 AND #4). Which is enough space to accommodate for the 8 infants, 12 toddlers, and 30 preschoolers respectively.

In the Infant room there is a total of one changing table (within arms reach of a sink), 5 cribs, and 1 sink (the one located next to the changing table) available for children's use. In the Toddler room there is one changing table (within arms reach of a sink), two sinks (one next to the changing table and one in the restroom), one urinal, and one toilet. In the Preschool rooms there is a total of 2 toilets and 3 sinks available (two sinks in the preschool room and one sinks in the bathroom). Per child to toilet and sink ratio, the number of toilets and sinks in each room will accommodate the requested capacity for each component.

Four of the sinks in the facility are portable sinks. CAB LPA advised the Facility Representative section 101239(e)(4) Fixtures, Furniture, Equipment and Supplies: All toilets, hand washing and bathing facilities shall be maintained in safe and sanitary operating condition. There is a separate private restroom for the staff. Children who become ill during the day will be isolated in the front office area and will utilize the staff restrooms. Individual measurements are recorded on the Capacity Worksheet (LIC 9024).

OUTDOOR ACTIVITY SPACE:
LPAs observed a total of two outdoor yards on site. The infant component will utilize one of the yards, the yard that is in front of the facility. During today's visit, LPAs observed that this infant yard has a 4 foot fence that is drilled into concrete and has openings that are appropriate for infants as infants are unable to exit through the bars. The entry to the infant fence has a magnet that keeps the door in place. Upon observation and inspection, LPAs found that the fence door is able to open if pushed. The facility representative stated that a bungee cord will be used by the staff to secure the gate during times of outdoor play. The staff will only use the bungee cord on the top portion of the fence which cannot be reached by the infant children. The infant yard has black mats on the ground to serve as cushioning for children in this yard as the ground is made of concrete.

The preschool and toddler components will use the yard located in the back of the facility. This yard is accessible via stairs that lead out to the yard via the preschool room. There is another route that can be used to access the yard which is via the elevator that is adjacent to the staff bathrooms outside. LPA Manabat discussed the importance of supervision at all times if this route is to be used and that the children should access the elevator with the staff and never alone. The Licensee Representative has submitted a rotating waiver request for the toddlers and preschool children to utilize the yard at a separate time.

The entire outdoor play area for preschool and toddlers is surrounded by a wire fence that is exactly 4 feet tall. LPAs observed trees that can be utilized for shade. The playground contains a substrate material made from wood chip so that there is cushioning for the children. There are currently two swings and a play dome that can be used by children. LPAs observed sufficient amount of age appropriate equipment and toys. Applicant was reminded that children are to use age appropriate toys and equipment for each of the groups that utilize the play yard.

Measurements:
LPAs measured a total of:
- 734.10 square feet in the infant yard
- 4,592.10 square feet in the toddler and preschool yard.
This will accommodate Applicant's request for 8 infants, 12 toddlers, and 30 preschoolers respectively.

LPAs discussed the following: 100% supervision is required at all times; personal rights; inspection authority; reporting requirements; staff to children ratios and capacity; staff qualifications; and maintaining buildings and grounds. LPAs discussed with Applicant any changes that may occur regarding the Director or an employee acting in the Director's absence must be reported to department within 10 working days.

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. Facility Representatives was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. (Report continued on subsequent LIC 809)
