At 1:10pm on 6/6/2025, Licensing Program Analyst (LPA) Matthew Gallo met with facility representative Amy Wright to deliver findings of a complaint investigation into the above allegation. Upon arrival, LPA observed census of 12 children consisting of 3 infants supervised by 2 staff, and 9 napping infants supervised by 1 staff.

Throughout the course of the investigation, LPA conducted observation and interview related to the allegation that staff do not properly safeguard the facility grounds in the case of an unstable play kitchen which fell on a toddler and a barricade to a wood burning stove that was not properly anchored to the wall. During LPA's visit on 3/20/2025, LPA observed the play kitchen in the toddler room to be stable in its position against a wall. Interviews with staff did not produce evidence that the kitchen fell on on any child, and parent interviews did not express any concerns with play equipment.

During the 3/20/2025 visit, LPA also observed the barricade to the wood burning stove, which is located in a recessed wall section. One end of the barricade was anchored to a wall, while the other end was positioned in front of another wall, impeding a child from walking straight into the recessed area to the wood burning stove. Interview with staff did not produce evidence that children in care have tampered with the barricade or bypassed it to access the wood burning stove. Upon LPA's visit on 6/6/2025, LPA observed the barricade to be fully anchored to the wall on both sides. The preponderance of evidence standard has not been met; therefore, the allegation is UNSUBSTANTIATED.

Exit interview conducted and report was reviewed with the facility representative, Amy Wright. A notice of site visit was given and must remain posted for 30 days. LPA provided facility representative with appeal rights.
