On 3/8/2024, at 9:00am Licensing Program Analyst (LPA) Anna Chan conducted an unannounced Complaint investigation inspection. This is to deliver findings of the investigation initiated on 1/24/2024. Upon arrival, the LPA met with Director Rebekah Bay. LPA informed director of the purpose of visit and LPA was led on a walkthrough of the facility and a census was taken. LPA observed 4 staff and 26 preschool children in the yard.

The Department received a complaint on 1/18/2024 alleging (1) Child sustained unexplained rash while in care (2) Staff left daycare child in soiled clothing.

LPA interviewed 6 staff at the facility. According to staff interviews, parents are notified through Procare app when children start to have a rash and staff also stated they apply diaper rash cream on affected area. Each child has their own diaper rash cream. Staff also stated that children have extra clothing in their cubbies in case accidents happen and they need to change clothes. Staff also stated that parents are informed when there are soiled clothing that need to be washed.

LPA reviewed children’s files and diaper log through Procare, the logs shows if a diaper is changed and diaper rash cream was applied.

LPA did a classroom observation after nap time and during diaper changing, staff logs each diaper change.

LPA interviewed children and none of the children interviewed disclosed any information that could support any of the allegations.

LPA interviewed parents and none of the three parents interviewed disclosed any information that could support any of the allegations.

Based on the interviews conducted, observation and record reviews, the preponderance of evidence standard has not been met. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.

An exit interview was conducted with Director Rebekah Bay. The Notice of Site Visit was posted during the visit. The director was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. The director was provided a copy of their appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. First-level appeals should be sent to the regional manager to the address listed above.
