32 | During the investigation, LPA Thompson interviewed facility representative Lisa Holloway. Lisa stated she currently has two ferrets inside her office that are rescues from Arizona. When ask if she had a current permit for the ferrets, Lisa Stated "no i do not." LPA observed two ferrets inside the office of the facility representative. Lisa mentioned children interacted with the ferrets through the fence when outside on the play yard maybe twice this past school year.
Based on LPA observation of two ferrets inside the office and interview with facility representative Lisa Holloway on 08/10/22, stating she does not have permits for the ferrets. The preponderance of evidence standard has been met, therefore the above allegation, staff maintain animals in the childcare that are illegal in California is found to be substantiated. California Code of Regulations, Title 22, Division 12 & Chapter 1, Section 101151(b) is being cited on the attached LIC 9099D.
Notice of Site Visit
A notice of site visit was given and must remain posted for 30 days.
Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
Exit interview conducted and report was reviewed with facility representative Lisa Holloway. Appeal Rights were explained, facility representative was provided a copy of her Appeal Rights and signature on this form acknowledges receipt of these rights. facility representative was informed all appeals must be in writing and received by the Regional Office within 15 business days. |
