During the investigation LPA toured the facility, interviewed three staff, reviewed video recordings, reviewed pictures, retrieved attendance sheets, and obtained the facility roster.
Reporting Party (RP) alleges, Staff left child in soiled pullup for an extended period of time. RP mentioned child #1 (C1) diaper was not changed all day. RP said that when C1 was picked up after school, C1 was still wearing the same pull-up since that morning. RP reported that the pull-up was soaked and C1 developed a rash from sitting in urine all day. RP added that last year, the staff logged diaper changes, but they no longer do so now that C1 is in the preschool classroom. On 02/25/2026, LPA interviewed three staff. Staff interviewed stated diapers are changed approximately every two hours or as needed depending on if the child has an accident. One staff member stated diapers for C1 are marked to show diaper changing is occurring. Staff stated C1 arrived to school with a pull-up but left the center at pick up with a diaper which is provided by the parents. Being that C1 is in the preschool program, the facility is not required to have a diaper changing log. On 02/25/2026, LPA reviewed pictures showing C1 diapers are being marked to confirm diaper changing. On 03/02/2026, LPA contacted eight parents via phone call. Out of eight parents called, LPA was able to interview three parents. Parents interviewed did not divulge any information pertaining to the allegation, staff left child in soiled pull-up for an extended period of time.
Reporting Party (RP) alleges, Staff did not report incident to authorized representative. RP mentioned that at pickup time on 02/18/2026, C1 had dry blood in and around their nose, as if C1 had been injured, and C1 clothes had been changed. RP also mentioned observing dry blood around C1 nose at pickup. On 02/25/2026, LPA interviewed three staff. Staff interviewed stated on 02/18/2026, pizza was given to the children for lunch. Staff stated C1 clothes were changed on 02/18/2026, because C1 had pizza sauce on their clothing. On 02/25/2026, LPA reviewed video footage from 02/18/2026, during the time C1 was leaving the facility. LPA did not observe any blood on C1 face and couldn’t determine if there was blood on or around C1 nose. On 03/02/2026, LPA contacted eight parents via phone call. Out of eight parents called, LPA was able to interview three parents. Parents interviewed did not divulge any information pertaining to the allegation, Staff did not report incident to authorized representative.
