25 | Licensing Program Analysts (LPAs) Rose Ruppert and Michael Tea conducted an unannounced visit. The purpose of today’s visit was to conduct the Annual Required inspection. LPAs Ruppert and Tea were greeted and granted entry into the facility by Administrator (AD) Loida Manguray and explained the reason for the visit. The facility is licensed for six non-ambulatory residents, with a hospice waiver for three. Currently there are four residents, two are on hospice during today's visit.
LPA reviewed two staff files and four resident records. Resident and staff files contained all required documentation. Administrator certificate expires on May 1, 2026. The last disaster drill was conducted on March 10, 2025. LPAs reminded the administrator that disaster drills need to be conducted quarterly, within 3 months.
LPA Tea along with AD Manguray toured the facility. LPA toured the physical plant, checked food service, and the first aid kit. The home consists of six resident bedrooms, one staff room, three full bathrooms, a living room, dining room, and kitchen. LPA observed smoke detectors/carbon monoxide in common areas and bedrooms are operational. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure and shower was free of mold/mildew. The water temperature measured 112.1 Fahrenheit degrees. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked at time of visit.
Common areas were clean and clear of hazards; doorways were free of obstructions. First aid kit had all the required elements including bandages, tweezers, thermometer, and scissors. Kitchen was inspected. Perishable and non-perishable food supply was checked and adequately stocked at the time of visit. LPA did observe expired non-perishable food in the pantry and advised AD Manguray to periodically check for expired ones and discard them.
