25 | On 12/6/2023, Licensing Program Analyst (LPA) A. Silva conducted an unannounced Required – 3 Year inspection assisted by licensee Amalia Osorio. Upon arrival, the licensee had three clients in care including two infants. The facility was operating within its licensed capacity and within compliance of staff-to-child ratio. An on-site Facility Personnel Report Summary review showed that all facility residents, staff, or other individuals who require background checks have received criminal record and child abuse index clearances or exemptions.
INDOOR INSPECTION. The LPA inspected the indoor day care area identified in the Facility Sketch LIC999 and areas accessible to clients (including the new additions). The LIC999 reflects the changes the licensee made as previously reported to the Department. The facility is a single-story home with 3 bedrooms, 2 bathrooms, 1 living room, 1 kitchen with dining room, a new dining room extension, a family room (label as “new working area” in the LIC999) adjacent to the new dining room, which leads to the back yard, a fenced front yard, and a fenced back yard.
OFF-LIMITS AREAS. The off-limits areas according to the LIC999 provided by the licensee include the following: The three bedrooms, the bathroom in the hallway that leads to the bedrooms, the garage.
DAYCARE ARES. The daycare areas as specified in the LIC999 include the following: The living room, the kitchen, the dining room areas (including the new dinning room extension), the new bathroom adjacent to the dinning room extension, the family room (new working area), the front and back yards.
The facility was equipped with working carbon monoxide and smoke detectors, working telephone service, and at least one fire extinguisher that meets statutory and State Fire Marshall standards. The licensee stated she keeps telephone service in the home when clients are in care. A record review shows that the last drill was documented on 10/24/23. |
