25 | On 10/25/2024 at 11:45am, Licensing Program Analyst (LPA), Jung, conducted an unannounced case management inspection to follow up on an incident that was self-reported to the Department. Upon arrival, LPA met with Director, Mary Zenk. LPA disclosed the purpose of the inspection and was led on a tour of the facility. There were 17 infant children, one (1) assistant director, and two (2) infant teachers present. Hours of operation are Monday through Friday 6:30 a.m. to 6:00 p.m.
On 10/18/24, facility reported to the Regional Office (RO) that Staff 1 (S1) was observed pushing down Child 1’s (C1) head onto their nap cot during nap time. S1 was also observed grabbing C1 by their overalls to remain on the cot. Parent of C1 observed the incident via the facility’s security cameras and communicated the incident to the facility administrative staff.
On 10/25/24, LPA conducted a case management inspection and interviewed two facility staff. LPA was provided with documentation pertinent to the investigation. Facility was cited one Technical Violation for Reporting Requirements as the facility did not notify the department of a director change within 10 days as required by child care regulation.
Based on the information obtained and interviews conducted, one Type A citation for Personal Rights is being cited on the LIC 809D. LPA informed Director, Mary Zenk, that this report dated 10/25/24 documents one (1) Type A citation. Type A citations shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. LPA informed the Director, Mary Zenk, to provide a copy of this licensing report dated 10/25/24 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director, Mary Zenk. |
