Regarding allegation (1) Facility staff left day care child unattended.During the interview, four (4) out of four (4) staff stated that Child #1 (C1) was left inside of the toddler classroom without supervision. Staff #1 (S1) stated that they heard C1 crying inside of the classroom from the playground area. S1 attempted to open the door to the toddler classroom but it was locked. Staff #2 (S2) stated that they went to the side entrance of the school to gain access to the toddler classroom. S2 stated that C1 was inside of the toddler classroom without supervision. Staff #3 (S3) stated that the camera footage reviewed showed C1 left unsupervised for a period of eight and a half minutes. Staff #4 (S4) stated that they were told by Staff #6 (S6) that a child was left alone in the classroom. This is an immediate risk to Health and safety to children in care and a violation of Providing Care and Supervision.
The Orange County Regional Child Care Licensing Office investigated the allegation that the facility staff left day care child unattended. Based on information gathered from LPA’s interviews, the preponderance of evidence standard has been met; therefore, the allegations are found to be SUBSTANTIATED, California Code of Regulations Title 22, Division 12 & Chapter 1, Section CCR for 101229(a)(1) Responsibility for Providing Care and Supervision.
LPA Meza informed Director, Chantel Hamrick that this report dated 10/07/2025 documented one (1) Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.
LPA Meza also informed the Director, Chantel Hamrick to provide a copy of this licensing report dated 10/07/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
Exit interview conducted and report was reviewed with the Director, Chantel Hamrick. A notice of site visit was given and must remain posted for 30 days.
