On 1/22/2024, Licensing Program Analyst (LPAs), Anna Chan and Karen Navar conducted an onsite inspection for the purpose of a 3-year random. LPAs and director, Cheyeme, Cazares toured the facility inside and outside and the floor and yard plan were verified. Census was taken in individual classrooms. The overall census observed was 5 infant staff and 10 infants.

During the inspection it was determined that the facility is operating within its licensed capacity and within compliance of staffing ratios. Facility hours are 7a.m.- 6p.m., Monday through Friday. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

During the inspection of the indoor activity space, items which could pose a danger to children (detergents, cleaning compounds, and medications) were observed to be stored out of the reach of children. Poisons/Hazardous Items are not kept on the premises. Parents bring food and bottles from home for infants and facility provides snacks for infants who can self-feed. Food prep areas were clean and sanitary. Food is properly stored. Menus were posted where they could be reviewed by parents. Floors, equipment, and furniture were clean and were observed to be in good repair and free of sharp edges. There is drinking water available to children indoors by the means of an Arrowhead water jug. Bottles and drinking cups are labeled with infant’s name on it. Children nap in cribs or on cots, and bedding is stored individually and washed daily in room 4 by staff. Room 3's bedding is taken home by parents at end of week or when needed. The facility has conducted an emergency drill within the past six months 09/19/2023. The facility has a working carbon monoxide detector and fire extinguisher. Facility met all posting requirements. The California Child Passenger Safety Law was posted by the entrance of the facility.

The outdoor activity space was inspected for compliance. The playground was enclosed by a fence at least four feet in height.The surface of the outdoor activity space was well maintained and free of hazards. The cushioning material is made of green grass turf around the climbing equipment, slide, and
other similar equipment appeared to be enough to absorbs falls. Drinking water in the outdoor activity space is provided by Arrowhead water jugs. The outdoor equipment and toys were in good repair and free of sharp edges. There are no bodies of water present at the facility. The facility grounds.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care
Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every
years after the date of the first test.

For childcare center licenses issued after July 1, 2022, the licensee shall test their water for
lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-
CCP).

LPA verified that the lead testing was completed in accordance to the Written Directives
outlined in PIN 21-21.1-CCP. And did not past the lead testing requirements. This poses as a health and safety risk to children and Type B citations will be issued.

Based on LPA’s observations, record reviews, and interviews the following violations were observed and are being cited in accordance with California Code of Regulations, Title 22, Division 12, Chapter 3. 6 Type B violations; Section 101419.2(b)(2) Infant Needs and Services Plan, 101429(a)(2)(B Responsibility for Providing Care and Supervision for Infants, Health and Safety Code 1596.8662(b)(1) Administration of Child Day Care Licensing,1596.8662(b)(1), HSC Lead Testing 1597.16(a)(1), and Lead Testing Written Directives 101700(b). See the attached LIC 809D

An Inspection and exit interview were completed with director. The report was reviewed and discussed. Appeal Rights and deficiencies were discussed. The facility representative was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days.

The facility representative was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. The “Notice of Site Visit” must be posted on or adjacent to the door. Failure to post will result in Civil Penalties of $100.00. Page 4 of 4
