On 8/22/24 at 1:00pm, Licensing Program Analyst (LPA) Sarah Garcia conducted a case management visit- deficiencies to address the received information regarding the facility operating out of ratio. This is a continuation of the report conducted on 8/1/24. LPA met with Briana Solorio, education manager. A walk through of the facility was conducted, and a census was taken. Total census was 20 infants and 11 staff.

LPA received documentation confirming that on 8/21/24 at approximately 8:3am the Room 3 had a ratio of 15 infants and 3 staff. Based on record review, it has been determined that the facility operated out of ratio on 8/21/24. According to Title 22, California Code of Regulations, Section 3, Chapter 4, (1) Type A deficiency will be cited today. See attached LIC 809D.

LPA Sarah Garcia informed facility representative, Briana Solario that this report dated 8/22/24 documents (1) Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Sarah Garcia informed facility representative, Briana Solario, to provide a copy of this licensing report dated 8/22/24 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Exit interview was conducted. The Notice of Site Visit was posted. Staff was advised the Notice of Site Visit must be posted for 30 days or $100 Civil Penalty will be assessed. Appeal rights provided.

Facility representative agreed to conduct a staff meeting, discuss ratios, and view video, write down what they learned. Facility representative agreed to submit replies, training agenda, and staff sign in sheet to LPA email by 5pm on 8/29/24.
