A case management inspection was conducted today by Licensing Program Analyst (LPA) Anna Chan who met with the Director, Vanessa Stahl. A self-reported incident by the facility was received at the regional office on 03/06/24. Upon arrival, LPA observed 5 staff present and 46 children in care.

A review of criminal record clearances indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

When Staff #1 (S1) and Staff # 2 (S2) were in the classroom doing group time, child #1 (C1) was dropped off, S2 left the room with another child going to another room. S1 stated when counting the children during transition to the toddler playground, S1 discovered that the count was 13 children and while counting the children S1 realized that child #2 (C2) was not there. S1 asked another staff, Staff # 3 (S3) who was in the other yard to check the room. S3 stated she was with her own class with another co-teacher and went to check the classroom of S1. S3 stated S3 found C1 alone and unsupervised in the classroom. S1 stated she had 13 children because C1’s attendance was not recorded in the Child Supervision Record (CSR) during group time and so they were out of ratio.

During this investigation, LPA interviewed 2 staff and the director. Documents gathered and reviewed were, CSR and Ratio training, CSR Guide, Threshold Transitions “SATs” guide, staff and children attendance dated 03/06/24, CSR for S1 classroom dated 03/06/2024, and 2 staff coaching form dated 03/08/2024

Based on the information gathered from the interviews conducted and record reviews, it was determined that the facility was out of ratio and a child was left unsupervised in the classroom.

The facility was not in compliance with the California Code of Regulations, Title 22, Division 12. The following violations were cited, Two Type A violations: 101216.3(a) Teacher-Child ratio and section 101229(a)(1) Responsibility for Providing Care and Supervision were issued today on the attached LIC 809D.

LPA Chan informed Director, Vanessa Stahl that this report dated 03/15/24 documents 2 Type A citations which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.

Also, LPA Chan informed the Director, Vanessa Stahl to provide a copy of this licensing report dated 03/15/24 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Exit interview conducted and report was reviewed with the Director, Vanessa Stahl. A notice of site visit was given and must remain posted for 30 days.



Appeal Rights and deficiencies were discussed. The facility representative was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days.
