25 | On 09/25/24, A case management inspection was conducted today by Licensing Program Analyst (LPA) Chan who met with the director Vanessa Stahl. Upon arrival there were 3 staff present and 57 children in care. Children were napping when LPA arrived.
A self-reported incident by the facility was received at the regional office on 09/24/2024, which states that on 09/23/24, a child (C1) was left alone and unsupervised in the classroom.
Based on staff interview, on 9/23/24, C1 was left in classroom #9 unattended around 4:15pm, when the rest of the class transitioned to the playground. Another parent found the child in the classroom and brought the child to the playground with the class. Director stated, C1 representative was immediately notified of the incident.
The facility was not in compliance with the California Code of Regulations, Title 22, Division 12, Section 101229(a)(1) Responsibility for Providing Care and Supervision and is being cited on the attached LIC809D. This is a repeat violation from a citation issued on 3/15/24 and an immediate civil penalty was assessed.
LPA Chan informed director Vanessa Stahl that this report dated 09/25/2024 for Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Chan informed Director Vanessa Stahl to provide a copy of this licensing report dated 09/25/2024 Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
Exit interview conducted and report was reviewed with the Director Vanessa Stahl. A notice of site visit was given and must remain posted for 30 days.
