32 | The Department received a complaint on 01/07/2026 alleging Staff did not properly store medication resulting in the medication being lost
During the investigation, LPA interviewed 8 staff members, Assistant Director, 3 children and 3 parents, obtained and reviewed children’s records.
Reporting Party (RP) stated the facility failed to properly store emergency rescue medication.
During staff interview, 8 of 8 staff stated they keep the medication in the office. Assistant Director (AD) stated they do not recall receiving medication for Child 1 (C1). AD stated they do not have an updated medication checklist.
On 2/10/26, Director stated they moved the medication box in the kitchen and currently, there are no medications for preschool children.
LPA reviewed children’s file, C1’s file did not indicate a prescription medication on Physician’s report.
LPA interviewed 3 parents and none of the parents interviewed disclosed any information that could support the allegation.
LPA interviewed 3 children and none of the children interviewed disclosed any information that could support the allegation.
LPA observed storage of medication in the kitchen where first aid supplies were. LPA did not observe any medications in the preschool classroom.
Based on the observation, interviews conducted and records reviewed, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged Staff did not properly store medication resulting in the medication being lost did or did not occur, therefore the allegation is UNSUBSTANTIATED.
Exit interview was conducted with Director Iris Antuna. Report and were discussed. The Notice of Site Visit was posted and must remain posted for 30 consecutive days. Appeal Rights were provided.
