25 | On 6/19/24 at 9:00 a.m., Licensing Program Analyst (LPA), Christine Jung conducted an unannounced case management inspection to follow up on an incident that was self-reported to the Department. Upon arrival, LPA met with Director, Mojgan Vasseghi. LPA disclosed the purpose of the inspection and was led on a tour of the facility. There were 46 children and 6 staff members present. Hours of operation are Monday through Friday 6:30 a.m. to 6:30 p.m.
A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
An unusual incident report (UIR) was submitted to the Department on 6/13/24 reporting an injury to Child 1 (C1). The facility reported that on 6/12/24 C1 sustained an injury while playing on the playground which required medical attention. Interviews were conducted with staff members and C1’s authorized representative. C1 could not be interviewed as they were no longer enrolled at the facility. Facility staff reported the incident to the Department in a timely manner. During today's inspection, LPA interviewed staff, reviewed files, and obtained documents pertinent to the UIR. If any deficiencies were observed, disclosed, or discovered during today's visit, they will be addressed and cited at a later date.
Due to insufficient information available at this time, the reported incident needs further investigation.
A notice of site visit was given and must remain posted for 30 days.
