of ratio". The reporting party reported out of ratio in preschool at around 7:30am observed one teacher with 13 kids. Teacher asked for help but no one came to help.
Today, LPA interviewed total of 6 staff who work in the Preschool and infants and toddlers classrooms.
LPA reviewed the Internal Kindercare documentation of Child Supervision Record (CSR) for the week of 4/20/2026. LPA observed Staff # 1 (S1) had 13 preschool children by themselves from 7:54 to 8:04 am when S2 arrived for additional help on the CSR dated 4/21/2026 . LPA verified the arrival time of the 13th child when signed in on the electronic application and observed 13th child was signed in at 7:54 am. Therefore, according to the documentation, S1 was by themselves with 13 preschool children for 10 minutes. This document was confirmed with S1, S2, and S3. S4, S5, and S6 did not have information regarding the allegation and did not report being out of ratio.
Based on the interviews with 6 staff, admission of S1, S2, S3 confirming the documentation of CSR and reviewing Child Supervision Record (CSR) dated 4/21/2026, it was determined the preponderance of evidence standard has been met, the allegation of “Staff are operating out of ratio" is found to be SUBSTANTIATED. The facility was in violation of teacher child ratio that one staff has been left alone with 13 preschool children on 4/21/2026 for 10 minutes in the morning. The teacher child ratio for preschool is 1:12
Please see LIC 809D on next page for details of the deficiency. A previous licensing report was issued on 2/3/2026 and 2/17/2026 giving notice of the same violation. Because the facility has been cited for repeating the same violation within 12 months, the following civil penalty shall be assessed until the violation is corrected. An immediate civil penalty of 250.00 is hereby assessed for the repeat violation.
A notice of site visit was given and must remain posted for 30 days.
