Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek investigated the above allegation. LPA met with assistant director, Savannah Garcia. Census was taken. There were 4 infants with one staff in the infant room and 9 toddlers with 3 staff members in the toddler room.

A review of staff records on this date indicated that all facility staff or other individuals who required caregiver background checks have received criminal record and child abuse index clearances or exemptions.

On 1/27/2026, the Orange County Childcare Office received a complaint alleging "Child has got diaper rashes due to poor care". The reporting party stated starting last week, child has got rashes with skin open. The reporting party believes the staff did not apply cream provided for the child.

Today, LPA interviewed 7 staff who have worked in the infant/toddler program. (S1, S2, S3, S4, S5, S6, and S7) that might have information regarding diaper changing and rashes on Child # 1 (C1))

S1 stated they change diaper for a child who has diaper rashes every hour and they log it. They also apply cream on the child as instructed by the child's representative. S1 stated the cream fades off on the next diaper changing. The cream gets absorbed and S1did not see any residue.
S2 confirmed that they apply cream for the rashes and they change the child's diaper every two hours or as needed. However, children with diaper rash are changed every hour and cream would apply.
S3 stated they have not changed C1 recently because they are in the infant room for a short time.
S4 stated the routine is for all the children to be diaper changed every two hours or as needed. If they see rashes on the child, they change the diaper every hour.
S5 reported the routine of diaper changing and logging for all the children.
S6 reported they changed C1 when working in the infant room. They noticed C1 had diaper rashes when came on Monday 1/26/2026. They borough it up to the child representative's attention. Cream applied on each diaper changing every hour. Cream application wears off until the next diaper change.
S7 reported they noticed rashes on C1 and they applied cream.
LPA also reviewed the changing diaper routine for C1 and noticed the date, time and application of cream was logged in for C1.

LPA also contacted 10 random parents. Out of 10 parents, 3 parents responded with no concern regarding the above allegation.

This agency has investigated the complaint alleging "Child has got diaper rashes due to poor care"., although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.

A notice of site visit was given and must remain posted for 30 days.

was given to the reporting party. The reporting party also reported the infant room is cold.
Today, LPA interviewed 7 staff who have worked in the infant/toddler program. (S1, S2, S3, S4, S5, S6, and S7 that might have information regarding the allegations.
S1 reported that they did not know anything about missing bottle. Regarding room being cold, they said they did not know the staff could control the thermostat.
S2 reported they did not know anything about missing bottle. They know that staff shall return any bottled or food for the child to the child's representative at the end of the day. S2 stated staff can change the thermostat which is located in the toddler room.
S3 did not have information regarding missing bottle for the child. S3 was wearing a jacket in the infant room showing the room was cold.
S4 stated they heard regarding missing child's bottle but they were not involved. Room gets cold but they did not know if they were allowed to touch the thermostat.
S5 reported they were not involved in the missing bottle. However, S5 noticed one bottle left in the refrigerator and not returned to the child's representative the next morning they worked in the infant room. Regarding room being cold, yes the room gets cold. They tried to change the thermostat but it went down.
S6 reported they did not have knowledge regarding missing the bottle. However S6 admitted on one occasion they forgot to return a child's bottle to the child's representative at the end of the day. S6 stated the room gets cold sometimes.
S7 reported they made sure all the bottles for C1 had been returned to the child's representative at the end of the day. However, S7 observed one bottle left in the refrigerator and was not given to the child's representative at the end of the day when S7 entered the infant room.
LPA observed upon arrival today at 8:00 am that the thermostat which controlled infant room showed 66 Degrees F while the outside temperature was 51 Degrees F.
Based on LPA’s interviews with 7 staff today, admitting of 3 staff (S5, S6, S7) that confirms bottle of children were not returned to the child's representative at the end of the day, and 4 staffs' admission (S3, S4, S5, S6) that the room gets cold, and LPA's observation of the thermostat today, it was determined the preponderance of evidence standard has been met, the allegation of 1) Child's bottle was not returned to the child's representative at the end of the day, 2) the infant room is cold, are found to be SUBSTANTIATED.
