On 03/06/2026, Licensing Program Analyst (LPA) D. Valdez Santana made an unannounced visit to New Horizon School Irvine for the purpose to deliver findings of a complaint received. Upon arrival, LPA was met by Director, Hanaa El-Ansary who was explained the reason for today’s visit. LPA was provided a tour of the facility; census was taken in individual classrooms, Room Yellow had 8 children & 1 staff, Room Orange had 8 children & 2 staff, Room Purple had 8 children & 2 staff, Room Green had 5 children & 2 staff, Room Red had 6 children & 3 staff and Room Blue had 6 children & 2 staff. The total census observed was 41 children and 12 staff at the time of inspection. Director stated they currently have 56 children enrolled in the preschool.

A review of the Facility Personnel Report Summary conducted on today’s date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

On 12/15/2025 a complaint was filed with the Licensing office stating, Staff do not ensure daycare center is free of insects and Staff do not ensure day care center is clean and sanitary. The reporting party (RP) stated that multiple children in the preschool classroom have been coming home with rashes, bites, and skin irritations believed to have occurred while at the daycare. RP stated that there are concerns that the presence of insects and inadequate cleaning, sanitation, and disinfecting practices—particularly involving carpets, bedding, nap mats, and other soft materials—may be related to these issues. RP stated that the facility may not be following proper sanitation and pest control practices necessary to ensure a safe and healthy environment for children.

During the course of investigation, LPA interviewed 6 staff members, 3 parents and 3 children.

Regarding the allegation, Staff do not ensure daycare center is free of insects. During the staff interviews, 6 out of 6 staff stated they have not witnessed a lot of children get insect bites at the facility. Staff #1 (S1) stated seeing mosquitoes but not a lot. Staff put zappers in all the classrooms; it was an added measure because prior to that there were bug repellant plugin inserts in all the classrooms. Additional measures the facility is taking to ensure the facility is free of insects is the facility has a contract to do pest control maintenance. The administration changed pest control companies that included a consultant for the mosquitoes. The custodian additionally also helps with regularly checking that there aren’t spiders or insects in the classrooms. Staff #2 (S2) stated the facility has taken additional measures out of an abundance of caution. Part of the school’s maintenance is getting the school sprayed for insects but due to a child’s adverse reaction they decided to hire additional pest control specifically to treat for mosquitoes. They hired a pest control consultant and ended up changing pest control companies. Staff #3 (S3) stated seeing mosquitoes outside but, in the classroom, only once in a while, only 2 or 3. S3’s students have not complained about any mosquito bites but is not sure in the other rooms. Staff #4 (S4) stated seeing one mosquito or a couple but not at the same time. The classrooms have zappers to help. Only 2-3 children that got bit and so did S4 but S4 wouldn’t say a lot of children have gotten bit by mosquitoes. One of the children had a bad reaction so staff have been keeping an extra eye. A parent got bracelets to help with that, and the child would wear it at school. For the child that had a bad reaction, it is uncertain if the child got the bite outside because staff had not really seen any in the classroom. Staff #5 (S5) stated seeing mosquitoes but not a lot of them just a normal amount not too many. S5 has not observed that children get a lot of insect bites at the facility or in S5’s class, no, kids come here with a bite but not from S5’s class.

Staff #6 (S6) stated that mosquitoes come and go, there was a season of mosquitoes, facility got zappers and it helped a lot with the mosquitoes and fruit flies. But now not really, S6 has not seen mosquitoes. S6 has not observed that children get a lot of insect bites at the facility and not at all, at least not in S6’s classroom. Child #2 (C2) stated only seeing mosquitoes at the facility, but it was before but not a lot only medium. C2 disclosed getting bit at home and once at school and once at the park or pool. C2 recalled two other children who got bit at school. Child #3 (C3) and Child #4 (C4) did not make any disclosures. Three (3) of three (3) parents interviewed did not disclose anything in relation to the allegation and had no issues or concerns with the daycare.

LPA’s observations during the inspection, while touring the facility, LPA Valdez Santana did a thorough inspection of the classrooms, Preschool Orange, Preschool Yellow, Preschool Green, Preschool Purple, Preschool Blue, and Preschool Red. LPA did not observe any mosquitoes or any other insects in any of the classrooms. LPA observed bug zappers in each classroom placed on top of high cabinetry. LPA also observed "Flying Insect Indoor Light Trap Plug-Ins" in each classroom, also out of reach of children.

Regarding the allegation, Staff do not ensure day care center is clean and sanitary, S1 disclosed they ensure the facility is clean, the facility has a custodian that cleans daily. The staff help maintain the facility clean by cleaning and sanitizing after every activity. Staff are instructed to clean high traffic areas, sweeping after meals, checking the bathrooms after every use. Staff follow the cleaning protocols that the facility used during Covid times. The custodian vacuums and mops at the end of the day. The bedding is laundered once a week. Individual bedding is now placed in individual bags to ensure that bedding is not touching each other. This is an extra layer staff are doing to ensure their bedding is clean. Additionally, once a year that floor is deep cleaned, staff do all that they can to ensure the school is clean and the children are safe. S2 stated staff still follow a lot of the Covid sanitation protocols and had extra sanitation cleaning in the classrooms over the winter break. The administration did all of this out of an overabundance of caution. S2 ordered new rugs for two classrooms, that were only about a year old but decided to order news ones anyway. The other classroom rugs were steamed and cleaned. S3 stated the teachers clean before snacks and meals; and sanitize the tables. The kids have to wash their hands, and staff wash the tables and shelves. Staff wash the toys, materials, and cots once a week. Staff put the children’s bedding in the Ziploc bags. The facility also has a custodian that cleans the classrooms at the end of the night. Teachers clean the tables and sweep. The custodian vacuums and mops the floors.

S4 stated staff clean, in preparation for the day and throughout the day after activities. If there is any accident, then staff will clean it immediately. S4 also cleans the sink. S4 cleans all the time. S5 stated the facility has a custodian who cleans the facility and staff do the routine; in morning S5 cleans the tables and sanitize them. Before the snacks and meals, staff sanitize the tables, the children also help clean the tables. S6 stated in S6’s classroom, S6 sets up the classroom, wipes the tables and chairs, if S6 cleaned the toys the days before then S6 ensures they are dry and ready to be put back in the classroom, staff wipe the toys and countertops. Now staff store the blankets individually in the Ziploc bags. The facility also has a custodian who ensures the school has what it needs. Three (3) out of three (3) children did not make any disclosures. Three (3) of three (3) parents interviewed made no disclosures regarding the allegation.

LPA’s observations during the inspection, while touring the facility, LPA did a thorough inspection of the classrooms, LPA did not observe classrooms: Preschool Orange, Preschool Yellow, Preschool Green, Preschool Purple, Preschool Blue, Preschool Red to be unclean or unsanitary. The tables and floors were clean. LPA observed that the cots were stored outside of the bathrooms and in the classrooms, staff said this was to reduce exposure to any possible moisture. Children's bedding was stored inside Ziploc bags with their names labeled on them.

During the course of the investigation LPA obtained a copy of the Children's Roster. LPA was emailed a copy of the Personnel Report and Parent Handbook. Interim Head of School also emailed LPA a copy of the pest control and sanitation invoices, treatments seem to be ongoing. The Reporting Party (RP) provided emails of the RP and the administration where they are reassuring that they are taking extra precautions such as cleaning, extra pest control maintenance, Covid cleaning protocols. RP also sent screenshots of Child #1’s teacher informing RP that C1 was bit. RP sent LPA screenshots of a parent group chat where parents are sharing some of their children were bit.

Based on LPA’s interviews and record review, there is insufficient evidence to corroborate the allegation of Staff do not ensure daycare center is free of insects and Staff do not ensure day care center is clean and sanitary. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur; therefore, the allegations are UNSUBSTANTIATED.

Exit interview was conducted, and report was reviewed and discussed with Director, Hanaa El-Ansary. Notice of Site Visit was posted during the visit. The facility representative was informed that the Notice of Site Visit must be posted for 30 consecutive days. The facility was provided a copy of their appeal rights (LIC 9058 12/15) and their signature on this form acknowledges receipt of these rights. End of Report. Page 4 of 4.
