On 12/08/2022 LPA Thompson arrived at the day care at 9:25 AM and was present until 4:00 PM. During LPA's visit, LPA toured the home inside and outside and observed eight children in care. Out of the eight children in care, one was child #1. At approximately 3:00 PM, LPA observed the mother pick up child #1 and the mother did not mention any injuries to the licensee Maria. LPA did not observe any children sustaining injuries while in care during the time LPA was present.
on 12/12/2022 When Maria Aldana was asked if she recalled any children sustaining injuries while in care on 12/8/2022, Maria replied "No."
On 12/19/2022 a telephone call was made to the parents of child#1 and a voicemail was left requesting a return phone call. On 12/22/2022 a telephone call made to the parents of child #1. Mother of child #1 requested not to receive calls from LPA.
On 12/19/2022 LPA received the Special Incident Report from Children Home Society per request.
On 1/03/2023 LPA sent an email to child #1 Social Worker to see if more information or pictures could be provided in regards to the allegation. on 01/05/2023 LPA was informed by child #1 Social Worker that no information was received pertaining to child #1 sustaining an injury on 12/08/2022 while in care at Maria Aldana family child care home.
Based on LPA’s facility inspection, observations, interviews conducted with licensee, and records reviewed it was determined there was insufficient evidence alleging Licensee did not prevent day care child from sustaining an injury. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is Unsubstantiated.
Exit interview was conducted with licensee Maria Aldana. Notice of Site Visit was posted during the visit. Licensee was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. Licensee was provided a copy of their appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. First level appeals should be sent to the regional manager to the address listed above.
