25 | On February 23, 2026, an unannounced annual random inspection was conducted at the facility by Licensing Program Analyst (LPA) Long Pham. At 2:25pm, LPA observed licensee Assadzadeh Mina and assistant Maria Devergaro providing care for 3 infants and 5 preschool children. Licensee was operating within the licensed capacity as specified on license.
A review of the Facility Personnel Report Summary on this date indicates all facility residents, staff, or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Currently, according to the Licensee, there are 2 adults including the licensee and 1 minor residing in the facility. Facility Day care hours are 7:30am-5:00pm, Monday through Friday.
During today’s inspection, LPA and licensee toured the inside and outside areas of the facility sketch as accessible to childcare children. This is a single-story home with 4 bedrooms, 3 bathrooms, a living room, a family room, a kitchen, a dining room, a garage, the front yard (not fenced), and the backyard (fenced). Licensee stated the day care area consists of 1 bedroom, 1 bathroom, the family room, and the backyard (fenced). Off-limit areas include 3 bedrooms, 2 bathrooms, the garage, the living room, the kitchen, and the front yard (not fenced). LPA verified the childcare areas on the facility sketch and off-limits are made inaccessible by means of baby gates. The facility has a fireplace that is barricaded by a metal cover. There is at least one working carbon monoxide, smoke detector, and fire extinguisher in the home that meet statutory and State Fire Marshall standards. Last fire/disaster drill documented on 2/13/2026. Detergents, cleaning compounds, medicines, and other items, which could pose a danger if readily available to children, were stored inaccessible to children. Licensee stated there are no firearms and/or other dangerous weapons in the facility, and none were observed during today's inspections.
