25 | On March 22, 2024 , an unannounced 3-year inspection was conducted at the facility by Licensing Program Analyst (LPA) V. Trinh. At 9:00 A.M observed licensee and 2 assistants Samari Rodriguez and Sunjha Woo, caring for total of 9 children, which included 4 infants were about to the a nap, 5 preschool were reading book with one of the teacher. The licensee was operating within the licensed capacity as specified on license.
A review of the Facility Personnel Report Summary on this date indicates all facility residents, staff, or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Currently there are 3 adults including the licensee and 1 minor child living in the facility. Facility Day care hours are 6am-6pm, Monday through Friday.
During today’s inspection, LPA and licensee toured the inside and outside areas of the facility sketch LIC 999 as accessible to childcare children. This is a three story home, with bedrooms and 4 bathrooms. Applicants have placed a safety gate at the bottom of the stairs leading to the second Second and third floor, ensuring the stairs and off limit areas are inaccessible to children in care, during operation hours. Off-limits areas are made inaccessible by means of baby gates or door-knobs. The licensee stated that OFF LIMITS areas include: 3 bedrooms, 3 bathrooms, a living room, a family room, Patio, a dining room, a laundry room, (second floor), and a garage.
One bedroom, one bathroom, dining room, family room, and balcony (located on the 2nd floor), the entire 3rd floor (two bedroom, two bathrooms, and laundry room. The licensee acknowledged that children may never enter these off-limit areas.
The childcare area designated the 1st floor (den, one bedroom, one bathroom), kitchen (located on the 2nd floor), and backyard, as part of the day-care. During today's inspection, each child was observed to have safe, healthful, and comfortable accommodations, furnishings, and equipment. Continued Page 2
