During the investigation LPA interviewed six staff, interviewed four parents, reviewed Incident/Accident Reports, reviewed Reported Timecards for staff, and Attendance sheets for preschool age children.
Reporting Party (RP) alleges Facility is operating out of ratio. RP mentioned they did not observe the facility being out of ratio but heard about the facility being out of ratio from a staff member. During todays visit on 05/20/2025 and on 05/08/2025, LPA observed the facility is operating within compliance of staffing ratios. On 05/08/2025, LPA interviewed six staff regarding the facility operating out of ratio. Staff interviewed all stated they have not been out of ratio or witnessed other staff being out of ratio. LPA obtained and reviewed Reported Timecards for staff for April 2025, and Attendance sheets for preschool age children for April 2025. On 05/15/2025, LPA contacted 12 parents via phone call. Out of 12 parents called, LPA was able to interview four parents. Parents interviewed did not divulge any information pertaining to the allege allegation, the facility is operating out of ratio. Based on the information gathered during interviews with staff, parents, and documentation; there is not enough preponderance of evidence to prove the alleged violation did or did not occur.
Reporting Party (RP) alleges Unqualified staff providing care and supervision to day-care children. RP mentioned staff #1 (S1) did not have proper credits to oversee the classroom. On 05/08/2025, LPA interviewed S1, S1 stated she did not have any credits and works at the center as a teacher’s aide. S1 stated the only time she has been left alone with children, is during nap times. An aide who is 18 years of age or older, and who meets the requirements of Sections 101216 and 101216.2, may supervise 24 napping children. S1 also mentioned, when children are awake, she is there with a fully qualified staff present. S1 provided the names of the staff members who are present inside the classroom that S1 oversees. During the visit on 05/08/2025, LPA reviewed staff files that were mentioned by S1 and found all staff to be fully qualified. On 05/08/2025, five staff stated they have only witnessed S1 caring for children alone during nap time. On 05/15/2025, LPA contacted 12 parents via phone call. Out of 12 parents called, LPA was able to interview four parents. Parents interviewed did not divulge any information pertaining to the allege allegation, unqualified staff providing care and supervision to day-care children. Based on the information gathered during interviews with staff, parents, and documentation; there is not enough preponderance of evidence to prove the alleged violation did or did not occur.
