25 | Licensing Program Analyst (LPA), V Trinh conducted an on site case management inspection for the purpose of investigating an incident that the facility self reported on 3/26/2024. A review of the Facility Personnel Report Summary conducted on 4/3/2024 indicates all facility staff or other individuals who required caregiver background checks have received criminal record and child abuse index clearances or exemptions.
LPA met with the Center Director Maryjoy Hilsabeck. There was a total of 43 preschool children with 4 staff in classroom. According to the incident report a child (C1) was left behind in the PreK classroom left alone during the transition time for approximately 7 minutes.
The incident involved a child being left unsupervised in a classroom, for 7 minutes.
During the course of the investigation, LPA interviewed Center Director Maryjoy Hilsabeck, and 2 staff, obtained a copy of the children's roster (LIC 9040), Employee’s hand books, incident report to parents, facility roster, sign-in/out copy of the day of the incident, and C1’s record, Monthly training for staffs, and Pre/K and T/K classrooms schedule.
At this time further investigation is needed. Exit interview conducted with Director Maryjoy Hilsabeck and notice of site visit must be posted at the entrance of the facility. Notice of site visit is to remain posted for no less than 30 days. Failure to post will result in civil penalties of $100. Appeal Rights was explained.
