On 11/02/23 , Licensing Program Analyst (LPA), Anna Chan conducted an onsite inspection for the purpose of an unannounced Annual Visit. LPA met with Team Lead Kaithlyn Smith and toured the facility inside and outside and the floor and yard plan were verified. Director Kelsie Nanez arrived the facility at 9:35am. The facility is located across 24hour Fitness gym in Anaheim Hills Festival. Census was taken in individual classrooms. The overall census observed was 50 preschool children and 5 staff (1st room has 11 children and 1 staff, 2nd room 9 children and 1 staff, 3rd room 20 children and 2 staff, 4th room 10 children and 1 staff). It was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. Facility hours are 6:30a.m.- 6:30p.m., Monday through Fridays.

A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

During the inspection of the indoor activity space, items which could pose a danger to children (detergents, cleaning compounds, and medications) were observed to be stored out of the reach of children. Poisons/Hazardous Items are not kept on the premises. Food is prepared on site; Food is brought from home and are also provided by the facility. Food prep areas were clean and sanitary. Food is properly stored; Menus were posted near the entrance walkway to the office and at the kitchen area where it could be seen by parents. Copy of menus are also sent to parents weekly by the facility thru Sprout about app. Allergy list of children are also posted in the kitchen and in the classrooms. Water is provided by a gallon of purified water and sports bottles with child name on it.

Children nap on cots/mats, and bedding is stored individually. Beddings are brought home weekly by children for washing. The facility has conducted an emergency drill on Sept 26, 2023. The facility has a working carbon monoxide detector and fire extinguisher. Facility met all posting requirement. The California Child Passenger Safety Law was posted by the entrance of the facility.

The outdoor activity space was inspected for compliance. The preschool playground was enclosed by a fence at least four feet in height. The surface of the outdoor activity space was well maintained and free of hazards. The cushioning material in the preschool playground is turf grass on one side and rubber playground surfacing on the climbing structure such as slide and other climbing equipment which appears to be enough to absorb falls. Drinking water in the outdoor activity space is provided by gallon of purified water and refillable sports bottles with the child’s name on it. The outdoor equipment and toys were in good repair and free of sharp edges. There are no bodies of water present at the facility, and none was observed during today’s visit.

4 Staff files were reviewed for staff present during the facility inspection on this date. Health screening and immunization as required were reviewed. Beginning September 1, 2016, Health and Safety (H&S) 1596.7995 (a)(1) a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. Proof of immunization against pertussis, measles for the facility were reviewed and 4 of 4 staff does not poses proof of immunization.

In addition, 2 of 4 staff do not poses proof of Health Screening and TB test

Beginning March 31, 2018, H&S Code 1596.8662 requires all directors and employees to complete mandated reporter training, and to renew the training every two years. At least one staff member present possesses current EMSA approved Pediatric CPR/First Aid certifications, which expires 10/2025. Currently only the Director has Pediatric CPR/First Aid certification

5 Children's records were reviewed, and there was a separate, complete and current record for each child. In the areas reviewed the children’s files were found to be in full compliance. Sign in/out procedure was reviewed for compliance, the facility uses Sprout About app for sign in and out. Each child representative has their own login ID.

CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES: Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

Currently there are no medications in the facility. This facility provides Incidental Medical Services (IMS) LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Based on LPAs observations, record reviews and interviews, the following 2 Type B violations were observed are being cited in accordance with California Code of Regulations, Title 22, Division 12, Section 101216(g)(1) Personnel Requirements and Health and Safety Code 1596.7995(a)(1) General Provisions and Definitions
An Inspection and exit interview were completed with Director, Kelsie Nanez. The report was reviewed and discussed. Appeal Rights and deficiencies were discussed. The facility representative was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days.

The facility representative was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. The “Notice of Site Visit” must be posted on or adjacent to the door. Failure to post will result in Civil Penalties of $100.00.
