32 | Director immediately met with S1 and S2 and verbally discussed the importance of signing in on Face-to- Name transition sheet, especially when a staff member is leaving the classroom or breaking a staff member. Director stated, staff will only be entering and exiting the classroom from front door and staff were reminded to sign in and out on Face to Name transition sheet when leaving the classroom and when staying in the classroom. Director stated, an all-staff training will be conducted on Saturday April 26, 2025, which management will discuss the importance of supervision, transitions, face to name, head count, and verbally communicate with one another during transitions.
Based on self-reported incident, and information gathered from LPA’s interviews and record review, it was determined the facility failed to provide visual supervision of a child in care, therefore, California Code of Regulations, Title 22, Division 12, Chapter 1 Section 101229(a)(1) Responsibility for Providing Care and Supervision is being cited on attached LIC 9099D.
LPA Duron informed Director, Jennifer Gardea that this report dated 3/24/2025 documents one type A citation. Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.
LPA Duron informed the Director, Jennifer Gardea to provide a copy of this licensing report dated 3/24/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
Exit interview was conducted. Notice of Site Visit was posted during the inspection. Facility representative was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. Director was provided a copy of their appeal rights LIC 9058 and their signature on this form acknowledges receipt of these rights.
