25 | Licensing Program Analysts (LPAs) Mila Quinto and Dianna Valdez Santana conducted an unannounced case management incident inspection in response to a self-reported Unusual Incident dated 8/26/2022. LPAs met with Director, Judy Coglianes and staff Jenn Belch. LPAs observed 8 school age children and 5 staff members.
A review of staff criminal clearance records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
On 8/26/2022 a self-reported Unusual Incident Report (UIR) was filed with the Licensing Office. The facility reported that a child ran out of the school grounds.
During today’s visit, LPAs interviewed 4 staff members. LPAs obtained a copy of staff roster and children’s roster.
Due to insufficient information at this time the Department will continue the investigation into this unusual incident report and will provide updates.
Exit interview was conducted. The Notice of Site Visit was posted. DIrector was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. |
