Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek conducted a case management inspection of the facility for the purpose of increasing the capacity of infant program from 16 children to 24 children by adding Room # 1 (will be named Room 2) to their existing Infant/Toddler Program. LPA met with director, Deanne Walkwitz who gave a tour of the school to LPA. Currently the facility is licensed for 16 infants and toddles ages 6 weeks to 24 months old in Room # 2 (will be named Room # 1). LPA observed there were 4 staff with 11 children in Room # 2 today. (will be named Room # 1). The facility is on the premises of The Evangelical Free Church of Orange.
A review of the Facility Personnel Report Summary indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
The facility has requested to provide care and supervision to 24 infants and toddlers ages 6 weeks to 24 months old, Monday to Friday from 7:00 am to 6:00 pm. The children are dropped off, signed in and out electronically at the entrance of the building to go to rooms. The facility has met all the posting requirements. Both rooms were measured.
The indoor activity space measurement for the current requested capacity is as follows:
Room # 2 (will be named Room # 1) is: 766.80 square feet. Crib area for napping children in this room which was 119.21 square feet was deducted from the activity area and was not included in the activity measurement.
Room # 1 (will be named Room # 2) is: 901.50 square feet.
Total indoor measurement is: 1568.30 divided by 35 square feet = 44.81 (49 children).

changing table is with raised sides (four sides) at least 3 inches. LPA discussed the positioning of the staff when changing diapers to make sure they have supervision on the other children while changing diapers. LPA discussed the procedures of diaper changing with director. She said the staff disinfect the diaper changing surface before and after each use and follow the procedure of diaper changing wearing gloves. LPA reminded the director to make sure staff never leave the child unattended on the diaper changing table.

Total outdoor activity space measurement is as follows:
Outdoor space for the infant/toddler playground is:1152 square feet divided by 75 = 15.36
(15 children). There is a stair case located in the middle of the outdoor space leading to upstairs which is barricaded with a metal gate. The director stated the staff never use the stairs and they use the other entrance to go upstaters if needed too. There is a waiver for playground usage on a staggered schedule for maximum of 12 children at any given time.
Total of toilets: 2 X 15 = 30 children.
Total of sinks: 4 X 15 = 60 children

The facility is equipped with carbon monoxide detector. All the classrooms have age appropriate toys and equipment. · Always place infants on their backs for sleeping
· Use only a tight-fitting sheet on the crib or play yard mattress
· Do not hang any items from the crib or above the crib
· Keep all items, including blankets, out of the crib or play yard
· Pacifiers may be used as long as they do not have items attached to them
· Infants should not be swaddled or have any items covering them while sleeping

· The temperature of the room should be comfortable enough for an adult to wear a T-shirt and not be too hot or too cold. The director was reminded of safe sleep regulations and logging every 15 minutes check on infants and toddlers under 24 months old. A sample of the log was given. LPA also discussed the items not allowed in the childcare facilities for infants and toddlers. A flyer of the items was given to director.

Orange county Fire authority has approved the facility for maximum of 24 infants and toddlers in Room # 1 and 2.


Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Exit interview conducted and report was reviewed with the director, Deanne Walkwitz.

Based on today’s indoor and outdoor measurements, the sinks and toilet, the request of the facility for adding another room for 24 children ages 6 weeks to 24 months old. ( The facility will keep 12 infants in room 1 and 12 toddlers in room 2) will be final reviewed and discussed with management in the Regional Office and after receiving the following. The facility will be notified if any additional information is required.

Please submit the following no later than 30 days of today's date.

1- Send a picture of crib area for napping children in infant room (Room # 1) after installing the gate and securing the other half of the patrician.


2- The plastic netting cover on the metal gate leading upstairs on the playground shall be replaced. . End of report
