32 | April and May of 2024. S5 stated that the shed was built a month and half ago and only worked on the shed during the weekends. S1 and S2 stated S5 worked on the shed during the week, Monday through Friday. S4 stated S5 worked on the shed while the children were napping. S1, S2, and S4 stated S5 did not worked on the shed when the children were present outside. 4 out of 4 staff members stated during the time the shed was being built the area was blocked with yellow tape and the children knew not to go to that area. S4 stated children do not play around the area where the shed is located. During the inspection dated 6/3/2024, LPA inspected the playground and observed the area where the shed was being built was blocked off with the yellow tape. LPA did not observe any hazardous items on the playground.
On 6/3/2024, LPA Nunez tried to interview 2 children, but the children did not qualify for an interview due to the children not being able to answer the qualification questions.
On 7/12/2024, LPA attempted to interview 8 parents however only 4 parents were available for interviews. The 4 parents were satisfied with the childcare center. LPA also spoke with C1’s Parent#2 (P2) and P2 stated they are very caring and are very happy with the school.
Based on LPA interviews conducted with 5 staff members, 4 parents, and records reviewed it was determined there was insufficient evidence that Staff handled children in a rough manner, Staff yelled at children, Staff spoke inappropriately to children, Staff left children in soiled diapers resulting in rashes, Staff not maintaining facility free from outdoor hazards. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are Unsubstantiated.
Exit interview was conducted with Administrato, Ranmal Jayasekara. Notice of Site Visit was posted during the visit. Director was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. Director was provided a copy of their appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. First level appeals should be sent to the regional manager to the address listed above.
