25 | Licensing Program Analyst (LPA) Thompson conducted an unannounced Case Management Inspection on today’s date. LPA met with Director Sandhya Sharan. Upon arrival, LPA observed 24 preschool age children with 3 staff.
During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions with the exception of Victoria Leininger.
LPA informed Director to contact Guardian to see why Victoria is not associated to the center. Director provided LPA with documentation from the department dated March 29, 2024, that shows Victoria association status as cleared.
Based on information provided, further investigation is needed.
In the areas that were evaluated, no deficiencies were observed of the California Code of Regulations, Title 22, Division 12 at the time of the visit.
Exit interview conducted and report was reviewed with Director Sandhya Sharan. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
Appeal Rights discussed. Director was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. |
