Child #1 (C1) was taken to the local emergency room and treated. Complaint states C1 was not changed often enough. Complaint also stated that as a request, C1 was to be fed 30 minutes after being dropped off, at 8:30am.
During the interview with Reporting Party (RP), they provided copies of C1’s daily reports from the facility’s electronic communication application dated 10/10/2023-10/13/2023 and 10/17/2023. The daily reports are updated throughout the day by staff members and provide details of the child’s feeding, diaper changes and nap schedule. RP states the staff can enter information on the app but are not actually performing the tasks. RP states C1 was enrolled at the facility for a week and was not being fed the food that was sent from home at the time requested. C1 was not interviewed as they are a non-verbal infant.
During LPA’s initial visit, the Director and six staff were interviewed. Regarding 1) diapering needs allegation: The Director stated that all staff are trained in diaper changing procedures upon hire. Director states procedures include all children’s diapers be checked and/or changed every hour as children are dropped off at staggered times, there is no set schedule. During the interviews with the six staff, 6 out of 6 staff stated they check and/or change children’s diapers every hour. 6 out of 6 staff stated they inform the children’s authorized caregivers verbally or on the facility’s app of any irritation observed during diaper changes. 3 out of 6 staff stated they informed RP of irritation observed and suggested different brands of medicated diaper rash creams.
LPA audited the daily reports for C1 for the seven days they attended on 10/09/2023-10/17/2023. 7 out of 7 days, the C1’s diapers were changed four to seven times a day. 7 out of 7 days, diaper cream was applied to C1 multiple times throughout the day during diaper changes. On 10/17/2023, C1’s last diaper check was at 4:12pm and they were described as dry. C1 was then picked up at 4:58pm, totaling a 46-minute time gap between C1’s last diaper check recorded and when C1 picked up.
During interviews regarding 2) feeding schedule allegation: 6 out of 6 staff stated if the child has a written feeding schedule, they will follow it. 6 out of 6 staff stated they will attempt to offer food brought from home. 6 out of 6 staff stated they do not falsely enter data in the facility app and have not observed staff falsify information being entered.
