On 2/06/2025 Licensing Program Analysts (LPAs) Karen Navar and Susan Deschampe conducted a Case Management Inspection to investigate previous deficiency which was cited during an investigation dated 2/03/2025. LPAs met with Director, Nicole Carreon. LPA observed 31 infants and 9 staff, 60 preschool children with 5 staff members, and afters chool program with 5 school age children and 1 staff.

During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

On 2/03/2024 facility was cited, California Code of Regulations, Title 22, Division 12, Chapter 1 Section 101416.5(b) Staff-Infant Ratio (b)There shall be a ratio of one teacher for every four infants in attendance. On 2/03/2024 at 9:25AM, LPA Navar observed 1 staff member (S1) providing care for 5 infants in the Infant classroom. S1 stated they need to call for back up. Type A citation was given. At 9:30AM Staff 2 (S2) arrived, and census was taken 6 Infants and 2 staff. (S1-S2).

LPAs Navar and Deschampe conducted this inspection to review facility’s procedures to ensure facility is following California Code of Regulations, Title 22 Regulations, Division 12. During inspection LPA took census and observed children’s files for LIC9224 Acknowledging Licensing Reports. LPA observed 2 LIC9224 forms had been signed since 2/03/2025.

Based on LPAs (observations, record reviews and interviews) the following violation(s) was/were observed in accordance with California Code of Regulations, Health and Safety Code-HSC, Section 1596.8595, is being cited on the attached LIC 809D.

Exit interview was conducted with Director Nicole Carreon. LPAs reviewed HSC1596.8595 Posting licensing report by childcare facility or home; duration of posting; civil penalty for failure to comply; reports to be provided to parents or guardian of each child receiving services. The facility representative was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. The “Notice of Site Visit” must be posted on or adjacent to the door. Failure to post will result in Civil Penalties of $100.00.
