On 08/26/25, Licensing Program Analyst (LPA) Olivia Meza conducted an unannounced visit to the facility to deliver findings for a complaint that was initiated on 06/20/2025. LPA met with Director, Loretta Munoz and Nicole Carreon and explained the purpose of the visit. Director Loretta Munoz led LPA on a tour of the facility and observed a total of 21 infants and thirteen (13) staff.

On 6/16/2025, the Orange County Regional Child Care Licensing Office received a complaint with two allegations: (1) Staff did not provide adequate food services for a daycare child, and (2) Staff did not ensure a daycare child was clean at all times.

During the investigation, documents were obtained, interviews were conducted with the reporting party, staff, and authorized representatives.

Regarding allegation (1) Staff did not provide adequate food services for a daycare child.

During interviews, Staff #1 (S1) stated, since Child #1(C1)’s parent did not provide milk bottles for C1 on 05/09/2025, facility staff assumed that Child #1 was transitioning from drinking milk to drinking water, hence, facility staff did not contact C1’s parents and provided facility’s lunch, snacks and water to C1 as scheduled meal, snacks and water as needed. S1 stated an apology letter was provided to C1’s parents when the concern was brought up about C1 not provided milk bottles. Due to the children in care are infants/non-verbal, LPA did not conduct children’s interview. Parents interviewed did not disclose any information to support the allegation. During the record review, LPA verified that C1 was provided lunch, snacks and water from C1’s 05/09/2025 Daily Report. Although lunch, snacks and water were provided to C1 on 05/09/025, it may or may not be adequate according to C1’s individual Needs and Services Plan.

Regarding allegation (2) Staff did not ensure a daycare child was clean at all times.

During interviews, five (5) out of five (5) interviewed staff stated if a child has soiled their clothes, the child will be changed immediately and confirmed they have not witnessed any infants not being changed when dirty or soiled. Interviewed staff stated facility staff is expected to check and change diaper every two hours and as needed. LPA verified that staff responses regarding diapering procedures are consistent with eight (8) step diapering procedures protocol from facility. Due to the children in care are infants/non-verbal, LPA did not conduct children’s interview. Parents interviewed did not disclose information to support the allegation. During record review LPA reviewed C1s Daily report on 05/09/2025 revealed that C1 was being changed every two hours and that diaper cream was applied as needed.

The Orange County Regional Child Care Licensing Office investigated allegations: (1) Staff did not provide
adequate food services for a daycare child (2) Staff did not ensure a daycare child was clean at all times is found to be UNSUBSTANTIATED.

Based on information gathered from LPA’s interviews and record reviews, although the allegations may have happened or are valid, there was not a preponderance of evidence to prove the alleged (1) Staff did not provide adequate food services for a daycare child is found to be and (2) Staff did not ensure a daycare child was clean at all times, did or did not occur; ; therefore, the listed allegations are found to be UNSUBSTANTIATED.

A notice of site visit was given and must remain posted for 30 days.
Exit interview conducted and report was reviewed with the Director, Lorreta Munoz.

During record reviews, LPA reviewed Needs and Services Plan (NSP) and Daily Reports for Child #1 (C1). The NSP of C1 reveals bottle feeding schedules as followed: “6am and every three hours. Keep offering every 30 minutes until bottle empties”. However, the Daily Report of C1 on 05/09/2025 showed no record of the bottle being provided to C1. The records reveal that the facility did not follow C1’s NSP.

The Orange County Regional Child Care Licensing Office investigated the allegation: Staff did not follow a daycare child's needs and services plan. Based on information gathered from LPA’s record review, the preponderance of evidence standard has been met; therefore, the allegation Staff did not follow a daycare child's needs and services plan is found to be SUBSTANTIATED, California Code of Regulations Title 22, Division 12 & Chapter 1, Section 101427 (c) Infant Care Food Service is being cited on the attached LIC 9099D.
