25 | Licensing Program Analyst (LPA) Valencia conducted an on site inspection. LPA and the facility representative toured the facility inside and outside, and the floor and yard plan (LIC 999) were verified. Census was taken in individual classrooms. The overall census observed was 53 preschool age children and 9 preschool staff. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Facility was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
During the inspection it was observed by LPA Valencia that a complaint report delivered to the facility on 8/9/22, by LPA Odom was not posted near the front of the facility visible to the public. This reported cited an A violation and is required to be visibly posted for 30 days from delivery of this report. This requirement was detailed in the report provided by LPA Odom, on 8/9/22. The facility was infomred of the Health&Safety Code 1596.8595 detailing this requirement. Based on these observations, the facility is being cited one B violation on the attached LIC809D page.
Appeal Rights and deficiencies were discussed. The facility representative was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. A notice of site visit was given and must remain posted for 30 days.
