On 2/03/2025 at 9:00 AM, Licensing Program Analyst (LPA), Karen Navar conducted an unannounced complaint inspection for the purpose of delivering findings. This is a continuation of a complaint inspection initiated on 11/26/2024.

Upon arrival LPA met with Director Nicole Carreon and was led on a tour of the facility both inside and outside. During inspection, LPA observed a total of 64 preschool children along with 11 staff.

A review of the Facility Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. During today’s inspection the facility was operating within its licensed capacity and within compliance of staffing ratios.

On 11/22/2024, the Orange County Child Care Office received a complaint alleging: 1) Broken water pipes on play yard. 2) Staff was really aggressive with a kid and thru them down on a cot.. Reporting Party (RP) stated there is a broken pipe somewhere on the playground and a staff was really aggressive with a kid and thru them down on a cot.

On 12/10/2024, the Orange County Child Care Office received additional information from RP regarding a child being thrown onto their bed while watching classroom camera in Tadpoles during nap time.

During the investigation, LPA toured the facility, conducted interviews, obtained a copy of Kiddie Academy Personnel Roster, Sign in/out Sheets, Maintenance List, Tadpoles Employee Policy and Procedure, Employee Diaper and Potty Training Policy and Procedure, Completed Diaper Changes, Completed Accident/Injury Reports, Attendance Summary by Classroom, Employee Time Card Approvals, Classroom Daily Attendance Sheets, Daily Chronological In/Out by Classroom, Kiddie Academy Parent Essentials Handbook, Child Care Facility Roster, Staff Document, Unusual Incident Report, and obtained video footage. Children had limited vocabulary did not qualify for interview.



Regarding allegation “Broken water pipes on play yard.” During visit, LPA observed playground grass area along brick wall of playground that had large puddles of stand still dark colored water. As LPA walked closer to the puddles of water LPAs shoes began to sink down into the muddy/grass. LPA observed the puddles to have small flies flying around the stand still dark colored water. LPA then walked to playground corner away from play structure and observed puddles of dark stand still colored water with small flies. As LPA got closer to puddles LPAs shoes sank into the muddy/grass surrounding the puddles of stand still dark colored water. LPA interviewed fourteen staff that stated the playground has had puddled muddy/grass for some time. Two (2) of thirteen (13) staff stated that they have had some parent concerns regarding children’s clothes coming home full of thick mud. Thirteen (13) staff interviewed stated that they try to keep children out of those areas by placing staff in those areas to re direct children away from the stand still dark colored puddled grass/mud and that they do not know why it is like that. Four (4) staff stated the facility is trying to fix it.

Regarding allegation “Staff was really aggressive with a kid and thru them down on a cot”. During visit, LPA interviewed 2 staff (S12-S13). 2 (two) staff stated that staff 14 (S14) went to support during nap time to give a teacher a break for their lunch and through video footage S14 had forcibly pulled down a child onto their cot. 2 (two) staff interviewed stated that there was no other staff in the room and that they reported it to Child Care Licensing as an Unusual Incident Report (UIR) on 12/11/2024. 2 (two) staff stated S14 no longer works at facility. LPA observed video footage of S14 grabbing child #2 (C2) by arm with one hand then placed their two hands under C2’s arm pit area and picked them up. C2’s feet came off ground and S14 swung C2 around and forcibly laid C2 on cot.

During parent interviews. LPA called 9 parents and LPA was able to interview 8 parents. The 8 interviewed parent did not divulge any information pertaining to the allegation.

Based on LPA observations, interviews, and records review, the preponderance of evidence has been met; therefore, the allegations of, broken water pipes on play yard and Staff was really aggressive with a kid and thru them down on a cot are substantiated. California Code of Regulations, Title 22, Division 12, Chapter 1 Section 101238.2 Outdoor Activity Space and Section 101223 Personal Rights Space are being cited. See LIC9099D for deficiency cited.



1 Type A violation is being cited. The director shall post and provide copies of the report to parents/guardians of the children in care at the facility by the next business day and shall provide to the parents/guardians of children newly enrolled at the facility during the next 12 months. In addition, the director shall immediately post upon receipt the Proof of Correction for 30 consecutive days and provide a copy to current and enrolling parents. The director is to keep Acknowledgement Receipt (LIC 9224) signed by parents in each child’s file. Failure to post Type A reports for 30 days will result in a Civil Penalty of $100.00

Exit interview conducted and report was reviewed with Director Nicole Carreon. The Director was informed that the “Notice of Site Visit” must be posted on or adjacent to the door for 30 days. Failure to post will result in a Civil Penalty of a $100.00.

On 11/22/2024, the Orange County Child Care Office received a complaint alleging: 1) Staff not changing diapers properly which cause child a diaper rash and blisters. 2) Parents are not informed regarding child injuries. 3) Facility is operating out of ratio. 4) Threatening to terminate a child due to retaliation. Reporting Party (RP) stated Child #1 (C1) gets diaper rashes due to staff not cleaning the child properly or frequently. Parents were never informed with what happened. RP stated a teacher was observed to be with 18 children and when any concerns are brought up to administrators, they threatened to disenroll the children.

During the investigation, LPA toured the facility on 11/26/2024, 12/11/2024, and 1/10/2025, conducted interviews, obtained a copy of Kiddie Academy Personnel Roster, Sign in/out Sheets, Maintenance List, Tadpoles Employee Policy and Procedure, Employee Diaper and Potty Training Policy and Procedure, Completed Diaper Changes, Completed Accident/Injury Reports, Attendance Summary by Classroom, Employee Time Card Approvals, Classroom Daily Attendance Sheets, Daily Chronological In/Out by Classroom, Kiddie Academy Parent Essentials Handbook, Child Care Facility Roster, Staff Document, Unusual Incident Report, and obtained video footage. Children had limited vocabulary did not qualify for interview.

Regarding allegation, “Staff not changing diapers properly which cause child a diaper rash and blisters.” During facility visits LPA inspected classrooms, changing table areas, and reviewed diaper changing logs in App. Tadpoles for children in care. During playground time on 12/11/2025 LPA spoke with Staff 10 (S10) that stated if a child needs a diaper change during playground time. That they are the one that normally that changes or takes a child to the bathroom. On 1/10/2025 LPA asked for a random sample of 3 children’s diaper logs for 3 days and sign in and out sheets. LPA interviewed 13 thirteen staff who stated there is diaper policy for Kiddie Academy and that they must document diaper changes in Tadpoles that parents have access to review.

Regarding allegation “Parents are not informed regarding child injuries”: During facility inspections, LPA toured facility and inspected classrooms and observed staff supervising and participating in classroom activities with children. LPA interviewed 14 staff who stated there is a Kiddie Academy Policy for injuries regarding children in care and that they fill out an incident report and it is given to administration in the office. 2 (two) of 13 (thirteen) staff stated teachers, admin, and parents sign the document.

All 13 (thirteen) staff stated that minor injuries are reported at the end of the day and that serious or head injuries are a phone call to parent by administration. LPA reviewed a random sample of the most recent accident/injury reports written and signed dated from 10/2024 2 (two) staff stated those are the most recent.

Regarding allegation “Facility is operating out of ratio.” LPA conducted inspections on 11/26/2025 at 11:55AM, 12/11/2025 at 9:10AM, and 1/10/2025 at 9:05AM. Upon arrival LPA toured the facility and took census and found facility in compliance with Title 22 101216.3 Teacher-Child Ratio. During facility inspection LPA interviewed 13 (thirteen) staff who stated that for preschool Ratio is 1-12 during classroom time and if that extra support is needed, they will call for support.

Regarding allegation “Threatening to terminate a child due to retaliation.” LPA interviewed 13 (thirteen) staff who stated that they have not heard of any parent concerns. 12 (twelve) staff stated they will refer parent/guardian concerns to administration for support. 2 (two) of 13 (thirteen) staff stated they will talk to the parent, investigate concern, and get back with them and have herd of parents being threatened.

During parent interviews. LPA called 9 nine parents and LPA was able to interview 8 parents. The 8 interviewed parents did not divulge any information pertaining to the allegation.

Based on LPA observations, interviews, and records review, the preponderance of evidence standard has not been met. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations of 1) Staff not changing diapers properly which cause child a diaper rash and blisters. 2) Parents are not informed regarding child injuries. 3) Facility is operating out of ratio. 4) Threatening to terminate a child due to retaliation. therefore, the allegations are UNSUBSTANTIATED.

Exit interview conducted and report was reviewed with Director Nicole Carreon. The Director was informed that the “Notice of Site Visit” must be posted on or adjacent to the door for 30 days. Failure to post will result in a Civil Penalty of a $100.00.
