32 | According to RP, the bruise was not there Monday night during bath time. RP believes the bruise occurred at the facility.During the course of investigation, LPA interviewed 4 staff members and 1 parent.
During the staff interviews, 4 out of 4 staff denied Child #1 sustained an injury while in care at the facility. Staff #1 (S1) stated the procedures for when a child gets hurt is first staff clean the injury, take care of the child, call the parents and write the ouch report. S1 writes the ouch report. Staff will observe the child for a period of time. Even if staff don’t see any evidence of an injury, staff still inform the parents. S1 stated child #1 (C1) did not sustain an injury while in care. S1 did not see any bruises so how do staff know the child did not get the bruise at home. Staff would have seen C1 fall, staff would have informed the parents even if there was no evidence of an injury. Staff #2 (S2) stated the procedures for when a child gets hurt is staff check and make sure if the child is okay, if there is any evidence of a child being hurt, S2 tries to find out how the child got hurt and if another child is involved. S2 treats the injury and lets S1 know. S2 does not know of any child or C1 sustaining an unknown injury while in care. Staff #3 (S3) stated the procedures for when a child gets hurt is staff will check the child, check their body, treat the injury and let S1 know, and S1 does the ouch report and tells the parents. S3 stated that S3 did not see C1 fall or sustain an injury.
4 out of 4 staff disclosed staff check the whole body for any additional injuries and ask the child if anything else hurts.
1 of 5 parents were interviewed, 1 of 1 parent interviewed had no issues or concerns with the daycare.
LPA obtained a copy of the Children’s Roster, Personnel Report, Parent Handbook and a copy of child #1’s file.
Based on LPA’s interviews and record review, there is insufficient evidence to corroborate the allegation of Child sustained unknown injury at the facility. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur; therefore, the allegation is UNSUBSTANTIATED.
Exit interview was conducted, and report was reviewed and discussed with Director, Madhuri Fernando. Notice of Site Visit was posted during the visit. The facility representative was informed that the Notice of Site Visit must be posted for 30 consecutive days.
