25 | Licensing Program Analyst (LPA) Tran conducted a case management inspection to follow up on a self-report unusual incident that was submitted to CCL Regional Licensing Office on 07/07/2023. It was reported by Tracy Nguyen, Director. During today's inspection, LPA Tran informed Director of the purpose of the visit. A tour of the facility was conducted, observed at the time of the visit was 48 preschool children with 6 staff members. A review of staff records on this date 08/22/2023, indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances.
According to the Director, the incident occurred on 07/06/2023 at approximately 6pm. Director stated that Staff # 1 (S1) signed out child #1 (C1) at approximately 5:55pm, when C1 was witnessed sitting at the bench in front of Director's office. At 6pm, S1 was getting ready to leave the facility with C1. Director walked out of the office to ensure front gate of the facility was locked. Within a couple of minutes, S1 realized that C1 was no longer on the bench in front of the Director office. Director and S1 went to look for C1 in all of the classrooms and facility grounds. C1 was no where to be found. Director called Garden Grove Police Department (GGPD) and GGPD arrived approximately at 6:30pm, and found C1 in Hope classroom, which adjacent to the Director's office. No Police Report was generated.
During today's inspection, LPA interviewed S1 and C1. S1 confirmed the details of the incident in which, C1 was signed out of the attendance and S1 was gathering belongings to leave the facility with S1. S1 walked away into a classroom and did not inform S1 nor response to S1 when S1 was calling for C1. When interviewed, C1 does not qualify for the interview.
Based on the information gathered from LPA's interviews with Director, S1 and C1, and record review of the sign in/out sheet as well as video footage, no deficiency was observed. LPA provided suggestions for facility Director and S1 to further avoid incident.
Exit interview was conducted. The Notice of Site Visit was posted. Director was advised the Notice of Site Visit must be posted for 30 days or 100$ Civil Penalty will be assessed. (End of Report) |
