During the investigation LPA interviewed one staff, reviewed 20 children files, and obtained the facility roster.
Reporting Party (RP) alleges, Licensee does not ensure all children enrolled have obtained all required immunization prior to enrollment. On 07/22/2025, LPA interviewed staff #1 (S1). S1 stated there were 80 children enrolled for the 2024 -2025 school year and currently has 72 children enrolled for the 2025 - 2026 school year. S1 mentioned the preschool has been closed since July 11, 2025, for summer break and only one new family has enrolled in June 2025. S1 stated children enrolled must have updated immunization records prior to enrollment. On 07/22/2025, LPA reviewed 20 children files that were currently enrolled and attended Heritage Christian Preschool. During file reviews, LPA observed children enrolled had the required immunization records prior to enrollment. Based on documentation reviewed and interview with S1; there is not enough preponderance of evidence to prove the alleged violation did or did not occur.

RP did not give specific names of children who were missing immunization records. On 07/22/2025, LPA interviewed staff #1 (S1). S1 stated all children enrolled have immunization records. S1 stated there were 80 children enrolled for the 2024 -2025 school year and currently has 72 children enrolled for the 2025 - 2026 school year. On 07/22/2025, LPA randomly reviewed 25 percent (20 children files) of the total enrollment for the 2024 – 2025 school year. LPA observed 25 percent of the children’s files reviewed had the required immunization records on file. Based on documentation reviewed and interview with S1; there is not enough preponderance of evidence to prove the alleged violation did or did not occur.
On 07/22/2025 and 07/25/2025, LPA conducted parent interviews via phone call. Out of 12 parents called, LPA was able to interview three parents. Parents interviewed did not divulge any information pertaining to the allegations.
