25 | On 4/23/25, a Case Management incident was conducted by Licensing Program Analyst (LPA) Olivia Meza who met with the Director, Fatima Asghar. The facility self-reported an incident to the Orange County Regional Office on 4/10/25 which stated that on 4/9/25, C1 (Child one) was served food to which they are allergic to. LPA Meza was lead on a walkthrough of the inside and outside of the facility. Census was 93 infant toddler children and 24 staff.
Based on interviews conducted, record review, and documents received; it was determined that C(1) was served food that they were allergic to.
The facility was not in compliance of the California Code of Regulations, Title 22, Division 12. 1 Type A violation was cited. See LIC809D for deficiency cited.
LPA Meza informed Director, Fatima Asghar that this report dated 04/23/25 documents 1 Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.
LPA Meza also informed the Director, Fatima Asghar, to provide a copy of this licensing report dated 04/23/25 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
Exit interview conducted and report was reviewed with the Director, Fatima Asghar. A notice of site visit was given and must remain posted for 30 days.
