25 | On 05/30/24 Licensing Program Analyst (LPA) Anna Chan conducted a case management deficiency report. LPA met with Director Amanda Gonzalez and informed the Director of the purpose of the visit. During today's visit, a tour of the facility was conducted. The overall census observed was 8 infant and 2 staff.
During a case management visit on 05/23/24, LPA observed one infant staff going out of the infant room to open the front door lobby. The infant room is about 15 steps to the left from the lobby entrance door. This happened twice around 11:10am and then another time, the same staff went to the kitchen leaving 8 infants with one staff in the infant room. 6 children were on the highchair doing activities while 2 infants were napping.
Director stated she will conduct a training with staff to ensure the situation will not happen again.
Based on observation and interview, the following deficiency was discussed and cited. The facility was not in compliance with the California Code of Regulations, Title 22, Division 12, 101229(a)(1) Responsibility for Providing Care and Supervision. Please refer to LIC809D for details of deficiency
Exit interview was conducted. Report and deficiency reviewed and discussed. Notice of Site Visit was posted during the visit. Director, Amanda Gonzalez was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. Appeal rights provided and explained. The Licensee was provided a copy of their appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. First level appeals should be sent to the regional manager to the address listed above.
