32 | During the investigation, LPA inspected the facility, interviewed 13 staff members, 6 children, 3 parents, and obtained a copy of the personnel report, children’s rosters, and the facility's parent handbook. LPA also reviewed children’s sign in and sign out sheets along with staff timecards for weeks: 2/24/25 – 2/28/25 and 3/3/25 – 3/8/25.
During the investigation, on 3/18/25 when LPA arrived at the facility, LPA observed the following: The 5yr old classroom had 1 staff member with 7 children. The 4yr old classroom had 2 staff members with 17 children. The 3yr old classroom had 1 staff member with 12 children. The 2yr old classroom had 3 staff members with 5 children. The 2yr old classroom had 1 staff member with 5 children. On 3/18/25 the facility was operating within ratio. On 5/13/25, LPA conducted an unannounced inspection and LPA observed the following: The 5yr old classroom had 2 staff members with 7 children. The 4yr old classroom had 2 staff members with 21 children. The 3yr old classroom had 1 staff member with 12 children. The 2yr old classroom had 2 staff members with 9 children. On 5/13/25, the facility was operating within ratio.
LPA reviewed children’s sign in and sign out sheets along with staff timecards for weeks: 2/24/25 – 2/28/25 and 3/3/25 – 3/8/25, which demonstrated that the facility was in compliance with the ratio requirement.
During the staff interview, all 12 staff members denied ever being over ratio.
On 5/14/2025, LPA was able to interview 3 parents, and the 3 parents did not have any concern regarding the allegations.
Based on staff members interviews and record review, the preponderance evidence of Facility is operating out of ratio has not been met. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur; therefore, the allegation is UNSUBSTANTIATED.
Exit interview was conducted, and report was reviewed and discussed with the Assistant Director,Daniela Valladolid . Notice of Site Visit was posted during the visit. The facility representative was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100 per day. The facility was provided a copy of their appeal rights (LIC 9058 12/15) and their signature on this form acknowledges receipt of these rights.
