During investigation, LPA interviewed 7 staff members and 4 parents. LPA also reviewed facility's diaper log and child's files..
During interviews, all interviewed staff stated they check diapers every 1-2 hours and document when they change the child's diaper. If there is any concern, staff will document the concerns in the facility's note book, inform Director and parent will be notified depending on the severity of the concern. When interviewed, Staff #1 (S1) stated S1 informed C1's representative during pick up that C1 had just woken up and had a full diaper. S1 asked C1's guardian to wait a few minutes while S1 changed C1's diaper, and was asked by C1's guardian not to change C1's diaper due to C1's guardian was in a hurry. C1's guardian asked S1 to give C1's guardian a clean diaper and the guardian will change diaper for C1. All interviewed parents provided positive feedback about the facility and had no concerns related to the allegations.
During record review, LPA reviewed child #1's and child #2's files, there were notes that C1 arrived to the facility with a full diaper and that C2 kept having bathroom accidents although C2 was already potty trained. Staff also has notes about frequent diaper changes, periodically ranging from 1.5 to 2 hours throughout the day for C1 and notes regarding C2's bathroom incidents in order to communicate with C1's and C2's guardian.
Based on the information gathered from LPA interviews conducted with 7 staff and 4 parents, record review of children's files, and facility's diaper log/notes, there is insufficient evidence to corroborate the allegations that Staff are not meeting children's toileting needs. Although the allegation may have happened or are valid, there is not a preponderance of evidence to prove Staff are not meeting children's toileting needs. There is also not a preponderance of evidence that the allegations did or did not occur, therefore the allegations are unsubstantiated.
Appeal Rights were explained. The Director was provided a copy of appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the regional manager to the address listed above. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director Adriana Agreda.
