On May 07, 2026, at 2:20pm, Licensing Program Analyst (LPA) Long Pham conducted an unannounced complaint inspection for the purpose of delivering findings. Upon arrival, LPA met with the director Agreda Flor Adriana and was led on a tour of the facility. LPA observed a total of 3 infants, and 2 staff. This is a continuation of a complaint inspection initiated on 03/16/2026.
A review of the Facility Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

During today’s inspection the facility was operating within its licensed capacity and within compliance of staffing ratios.

On 03/12/2026, the Orange County Child Care Office received a complaint alleging (1) Staff hit child in care and (2) Staff yells at children in care.

Reporting Party (RP) stated that RP observed Staff #5 (S5) hit the child #1 (C1) on the head with a toy. The RP stated C1 had been playing and hit another child (name unknown) with the toy, after which S5 grabbed the toy and struck C1 on the head. The RP also reported that S5 was frequently in the classrooms yelling at children, regardless of whether they are in trouble or not. The LPA attempted to contact the RP but did not receive a response.

During the investigation, LPA toured the facility, interviewed 5 staff, 5 parents, and reviewed pertinent documents. LPA did not interview the children because they are non-verbal.

Regarding the allegation of “Staff hit child in care.”

During the staff interview, 5 out of 5 staff denied the allegation of “Staff hit child in care.” Staff #1 (S1), staff #2 (S2), staff #3 (S3), staff #4 (S4) stated that they had never seen S5 strikes C1 on the head with a toy or hit any children in the classroom. S5 stated S5 never strikes C1 on the head with a toy. All 5 staff stated they never hit any children in care or observe any staff hit any children at all.
During the physical plant inspection today, 03/16/2026, LPA didn’t observe any staff or S5 hits any children in care.

Regarding the allegation of “Staff yells at children in care.”

During the staff interview, 5 out of 5 staff denied the allegation of “Staff yells at children in care.” Staff #1 (S1), staff #2 (S2), staff #3 (S3), staff #4 (S4) stated that they never seen S5 yells at any children in care. S5 also stated that S5 never yells at any children in care. All 5 staff stated they never yell at any children in care and talk to them softly. Plus, they only speak English to the children and never in a different language.

During the physical plant inspection today, 03/16/2026, LPA didn’t observe any staff or S5 yells at children in care.

During the parent interview, all 5 interviewed parents did not divulge any information pertaining to the allegations or express any concerns regarding care of the children.

Based on LPA’s observations, and interviews, the preponderance evidence of (1) Staff hit child in care and (2) Staff yells at children in care has not been met. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegations are Unsubstantiated.

Exit interview was conducted and report was reviewed with the facility representative Agreda Flor Adriana.

Notice of Site Visit was posted during the visit. The facility representative was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100.

Appeal Rights were explained. The facility representative was provided with a copy of the appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the regional manager to the address listed above.
