| State of Emergency COVID-19 tele-visit was conducted through Facetime.
Licensing Program Analyst (LPA) Yesenia Villa conducted an unannounced case management inspection to provide new findings related to a complaint finding on 11/19/2020. LPA Villa met with director, Jenny Marvel who guided LPA Villa on a tour of the facility.
LPA Villa and Director Jenny Marvel conducted a virtual walkthrough of the facility in all five classrooms. Classroom 101 had 10 preschool children present with one staff, classroom 102 had 7 children present with one staff, classroom 103 had 7 children present with one staff, classroom 104 had 5 children present with one staff and classroom 105 had 7 children present with one staff. A review of staff criminal clearance records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. The facility was observed to be in compliance of staff and children ratios.
LPA Villa provided a copy of the amended report to Director Jenny Marvel via email. The report was amended after an appeal overruled the finding of the SUBSTANTIATED complaint to UNSUBSTANTIATED. There were no citations issued for the complaint received on 02/26/2020 regarding allegation of staff not supervising children while in the bathroom.
An exit interview was conducted with Director Jenny Marvel Exit interview was conducted with Director Jenny Marvel via Tele-Inspection. A copy of the report along with Appeal Rights will be emailed to Licensee with a Read Receipt to acknowledge report was received. Licensee was asked to respond to email by copying the following, “I have read and received the Investigation Report and Appeal Rights, I acknowledge receipt.”
