On 10/03/23, Licensing Program Analyst (LPA), Anna Chan conducted an unannounced onsite inspection for the purpose of an Annual Visit. LPA met sub teacher Diana Munoz Ramos. Site Supervisor Yessica Hernandez arrived around 2:40pm together with the children that was picked up from school. LPA and Site Supervisor toured the facility inside and outside and the floor and yard plan were verified. The facility is located at the Benjamin Beswick Elementary using 2 portable classrooms, Classroom #1 and Room #2. At 2:25pm, there were no children present when LPA arrived. Children start arriving from school around 2:40pm at Room #1 and #2. Census was taken at 2:45pm, there are 27 children and 5 staff.

It was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. Facility hours are 7:00a.m.- 6:00p.m., Monday through Friday. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

During the inspection of the indoor activity space, items which could pose a danger to children (detergents, cleaning compounds, and medications) were observed to be stored out of the reach of children. Poisons/Hazardous Items are not kept on the premises. Food is prepared on site; “Super Snack” are provided through a third-party vendor called Great American Lunch. Food prep areas were clean and sanitary. Food is properly stored. Menus were posted in the kitchen areas and Room # 1 and 2 parent areas. Menus are also given to parents every month.

There is a waiver to use a shared restrooms with the Elementary School. The children's bathrooms are clean and sanitary

Floors, equipment, and furniture were clean and were observed to be in good repair and free of sharp edges. There is drinking water available to children indoors by filtered water dispenser and disposable cups. And children have their own sports bottles properly labeled with their names. The facility has conducted an emergency drill within the past six months in August 2023. According to Site Supervisor, the disaster drill was done at the Sycamore location since this location was under construction in August. The facility has a working carbon monoxide detector and fire extinguisher. Facility met all posting requirement. The California Child Passenger Safety Law was posted by the entrance of the facility.

4 Staff files were reviewed for staff present during the facility inspection. Health screening and immunization as required were reviewed. Beginning September 1, 2016, Health and Safety (H&S) 1597.622 states, a person shall not be employees or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles. Proof of immunization against pertussis, measles for the staff were reviewed and within compliance. Beginning March 31, 2018, H&S Code 1596.8662 requires all directors and employees to complete mandated reporting training, and to renew the training every two years. At least one staff member present possesses current EMSA approved Pediatric CPR/First Aid certifications, which expires 04/2025

Children's records were reviewed, and there was a separate, complete, and current record for each child. In the areas reviewed, 5 children’s files were found to be in full compliance. Sign in/out at the gate upon entrance and through an app called The Hubbe and was reviewed for compliance. The person who signs the child in and out uses their full legal signature and records the time of the day.

This facility provides Incidental Medical Services (IMS) LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. There is a waiver to share the playground with the Elementary School on rotation basis. The outdoor activity space was inspected for compliance. The playground was enclosed by a fence at least four feet in height. The surface of the outdoor activity space was well maintained and free of hazards. The cushioning is wood chips around the climbing equipment, such slides and other similar equipment appeared to be enough to absorbs falls. Drinking water in the outdoor activity space is provided by water fountain. The outdoor equipment and toys were in good repair and free of sharp edges. There are no bodies of water present at the facility. The facility ground were safe, sanitary and in good repair.

In the areas that were evaluated, no deficiencies were observed of the California Code of Regulations, Title 22, Division 12 at the time of the visit.

An Inspection and exit interview was completed with Site Supervisor, Yessica Hernandez. The report was reviewed and discussed. Appeal Rights were discussed. The facility representative was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days.

The facility representative was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. The “Notice of Site Visit” must be posted on or adjacent to the door. Failure to post will result in Civil Penalties of $100.00.
