On 12/15/2025 Licensing Program Analyst (LPA) K. Navar conducted an unannounced Case Management – Incident visit to investigate an unusual incident. Upon arrival, the LPA met with Site Supervisor Yessica Hernandez and informed them of the purpose of the visit. A review of the Facility Personnel Report Summary shows all facility staff or individuals who require caregiver background checks have received a criminal record clearance and a child abuse index clearance or an exemption clearance. The census at the time of the visit was 0 children and 4 staff present.

The Department received a self- reported incident report on 12/05/2025 stating that on 12/04/25 Child #1 (C1) had gone back into the classroom after staff and children had exited out of the classroom to go to the restroom located on Beswick Elementary main campus.

On 12/15/2025, the LPA interviewed 2 (two) staff (S1-S2), who were supervising children during the incident that took place on 12/04/2025. 2 (two) staff stated that 18 children and 2 (two) staff were transitioning to the restrooms located on Beswick Elementary main campus and that C1 was left in room 2. All staff interviewed stated that S1 was outside the classrooms and saw C1 in room 2 through the window and immediately went to C1 and took C1 to S2 who was at the restroom area on Beswick Elementary main campus. All staff interviewed stated that children were counted before transitioning out of room 2 and that the classroom door was left open and one light was turned off and one light on and that children were counted at arrival to the restroom located on Beswick Elementary main campus and that S2 then knew C1 was no longer with them and then immediately walkie S1 to inform S1 that C1 was not with them but S1 was walking up to S2 with C1 at that moment.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

During inspection on 12/15/2025 LPA and Site Supervisor walked to the restroom located on Beswick Elementary grounds main campus. During observation from room 2 at Catalyst kids facility to the bathroom located on Beswick Elementary main grounds, children would need to walk down a ramp located outside room 2’s door that leads to a gate located on Catalyst Kids grounds, then walk through the gate about 15 steps to a second gate that leads to a long path along a chain link fence, then turn right and walk down another long path that leads to the bathrooms. LPA and Site Supervisor timed the walk to be 1 minute 43 seconds away from Catalyst kids room #2 to the bathrooms located on Beswick Elementary main campus that children use.

On 12/15/205, the LPA obtained the following documents. A staff meeting agenda documenting protocols for supervision and have requested to be referred to Technical Support (TSP) services for additional resources.



Based on the LPA’s observations and interviews. C1 was left in room 2 alone without staff supervision for about 3-5 minutes . Therefore, the following deficiency of Title 22 Division 12 1 (one) Type A citation is cited: CCR 101229(a)(1) Responsibility for Providing Care and Supervision (SEE LIC809D ATTACHED)

LPA K. Navar that this licensing report dated 12/15/2025 documents one “Type A” citation. Type A citation(s) must be posted for 30 consecutive days during the hours that children are in care as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. LPA K. Navar further informed Site Supervisor that a copy of this licensing report must be provided to parents or guardians of all clients currently enrolled by the next business day or by the next day the children are in care, a copy of this report must be provided to the parents or guardians of all newly enrolled clients for 12 months from the date of this report, and signed Acknowledgement of Receipt of Licensing Report (LIC 9224) form, or another written equivalent statement, must be placed in the child's file for verification of receipt of the report.

An exit interview was conducted with Site Supervisor Yessica Hernandez. The Notice of Site Visit was posted during the visit. The director was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100.
