On 04/06/2023 LPA and LPM observed 3 infants with 2 staff. LPA and LPM observed 2 staff in the activity area playing with 3 infants. On 05/03/2023 there were 6 infants with 3 staff. LPA observed 2 staff feeding 3 infants and 1 staff supervising 3 napping infants in the crib area. LPA and LPM observed facility staff maintain adequate supervision and staffing/ratio.
During the course of investigation, LPA and Licensing Program Manager (LPM) interviewed 3 staff. LPA interviewed 6 parents. LPA unable to interview infants due to age and non-verbal.
During the interviews all staff exhibited knowledge of supervision and ration regulations. 3 out of 3 interviewed staff denied there has been any lack of supervision or ratio problems and express they didn’t have any concerns of other staff. During all facility inspections LPA observed the facility to be in ratio and supervision was appropriate.
LPA Lucero attempted to contact 11 parents by phone and was able to interview 6 parents. All interviewed parents stated they did not have any concerns with the supervision or care of facility.
Based on the information gathered from LPA's interviews with staff and parents, observations of ratio and supervision, and reviewing records, there is insufficient evidence to corroborate the allegations of (1) facility staff leave day care children unsupervised while in care and (2) staff do not provide adequate care to day care children in care. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the allegations did or did not occur in the day care facility, therefore the allegations are UNSUBSTANTIATED.
An exit interview was completed. The report was reviewed and discussed with Teacher Vivian Maeda. The Facility Representative was provided a copy of their appeal rights and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Licensing office within 15 business days.
The Facility Representative was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. Failure to post will result in Civil Penalties of $100.00. The “Notice of Site Visit” must be posted on or adjacent to the door.
