Licensing Program Analyst (LPA) Mila Quinto conducted a case management visit on 4/26/2021 due to an Unusual Incident Report (UIR) received regarding personal rights in the Regional Office on 4/20/2021 by Director Gloriette Cortez. A virtual tour was conducted by the director and present were 23 children and 5 staff members. The Covid-19 Emergency Response questionnaires were asked.
A review of facility Personnel Report Summary on this date (4/26/21) indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
During the tele-inspection, LPA discussed the unusual incident report with director regarding the alleged disclosure of staff member hit the child. On 4/26/21, 2 staff members were interviewed and described the child as well mannered, observant, quiet and likes to repeat what teachers say. The child did not have any behavioral issues and they were surprised on the child’s disclosure of staff had allegedly hit the child. Other parties involved were also interviewed and indicated they do not believe the child was hit by a staff member and stated the child had limited vocabulary.
Based on the interviews conducted, there is not a preponderance of evidence to support the child's disclosure. Therefore, no further action.
Exit interview was conducted, Report was read, and Appeal Rights were explained. A copy of the report along with Appeal Rights will be emailed to Director with a Read Receipt requested to acknowledge report was received. Director was asked to respond to email by copying and pasting “I have read and received the Report and Appeal Rights, I acknowledge receipt.”
