the week of 1/29/24 and sign in/out sheet for children. LPA checked the staff time cards for signing in and out and checked the number of children signed in and out and found the number of children and number of staff met the teacher child ratio. However there was not precise evidence to prove the location of any staff during their time card hours.
Regarding the allegation of "Staff did not meet infants' diapering needs causing a rash to infants in care", LPA interviewed 6 staff who work in the infant room. They stated they have not observed any diaper rashes on the infants. Two staff stated they have observed slightly rashes after a weekend on some children that tells them they came with rashes from home. Staff stated they use diaper cream on children when changing diaper. They check their diaper as needed to make sure they don't stay in soiled diaper. One staff stated they check every 20 minutes, one staff stated they check every hour, and one staff stated they check children's diaper every 2 hours. They all stated they have a daily log for children that they log it for feeding and changing diaper times and they give the log to parents at the end of the day and they don't keep a copy at school. Parents who were interviewed did not have any concerns related to this allegation.
Based on the interviews conducted with a total of 6 staff working in infant room, and contact of 8 parents of whom 6 responded, and LPA's today's observation and 2/1/2024, there is not enough proof or evidence to support the above allegations occurred or did not occur. This agency has investigated the complaint alleging "Staff left infants unsupervised, staff are not meeting staff-infants ratios, and staff did not meet infants' diapering needs causing a rash to infants in care; although the allegations may have happened or are valid, there is not a preponderance of evidence to prove, the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.
Notice of Site Visit was posted. The notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. The director, Saeideh (Fay) Gharavi was provided a copy of their appeal right (LIC 9058 1/16) and their signature on this form acknowledges receipt of these rights. No deficiency cited on this inspection date.
