On 11/05/25, at 1:45pm Licensing Program Analyst (LPA), Anna Chan conducted an 3-yr Required inspection. LPA met with Director Celida Cambuston. The license has been inactive since 2023. Licensee has requested to be active again. LPA and Director toured the facility inside and outside and the floor and yard plan were verified. There were no children present at the time of visit. This will be an after school program.

Facility hours are 6:30a.m.- 6:00p.m., Monday through Friday.

During the inspection of the indoor activity space, Room #6, items which could pose a danger to children (detergents, cleaning compounds, and medications) were observed to be stored out of the reach of children. Age-appropriate materials are provided. PM snacks will be provided by the facility. Food is not being prepared in the facility. Only dry snacks will be served. Floors, equipment, and furniture were clean and were observed to be in good repair and free of sharp edges. Water is provided by system filtration, and Sports bottles are also brought by children from home with the child’s name on it. The facility will be having an event this week, shelves in the school-age room are being covered with black cloth. Director will provide LPA photos of shelves when the event concludes.

The children's restroom is a jack and jill restroom with room #5. Director stated they will put a barrier between room 5 and 6. Proof of barrier will be sent to LPA. The restroom was inspected and was free of any hazardous items. There were 5 toilets and 4 sinks. The children's bathrooms are clean and sanitary.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Children will not nap at the facility. The facility has a working carbon monoxide detector and fire extinguisher. Facility met all posting requirement. The California Child Passenger Safety Law was posted by the entrance of the facility.

The outdoor activity space was inspected for compliance. The playground was enclosed by a fence at least four feet in height. The surface of the outdoor activity space was well maintained and free of hazards. The cushioning material used, sand and grass appeared to be enough to absorb falls. Drinking water will be provided by refillable sports bottles with the child’s name on it. The outdoor equipment and toys were in good repair and free of sharp edges. The facility has a shared playground waiver between the preschool and school-age.

There are bodies of water present at the facility. An In-ground pool and a separate spa was observed during inspection. The gate self-latches and self-closes and swings away from the pool. Bodies of water checklist was conducted. Director and LPA were able to test the working alarm. LPA measured the vertical clearance from the ground to the bottom of the enclosure, and measurement showed less than 2 inches vertical gap from the ground. LPA reviewed daily pool log. There was a life ring with a diameter of 17”, a rescue pole with a body hook and a minimum of fixed length of 12'.

Director stated they will have a current roster of children in care. Fire Drill log was available. Director was reminded fire drill shall be conducted every 6 months. Children's files and staff files were not reviewed due to not having any children enrolled yet because the license was inactive. The enrollment packet was reviewed with director. The director's Pediatric CPR/First Aid certification is current and expires 09/2027.

CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES:
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. Facility conducted Lead Testing on November 7, 2023.

Transfer Request (LIC9182). The Director was informed that Licensing Quarterly Updates are available at www.ccld.ca.gov and may request to be added to an email list to receive a Quarterly Update by contacting the Child Care Advocate at childcareadvocatesprogram@dss.ca.gov or at www.ccld.ca.gov

Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

In the areas evaluated no deficiencies were observed in the California Code of Regulations, Title 22, Division 12 at the time of visit.

An Inspection and exit interview were completed with Director Celida Cambuston. The report was reviewed and discussed. “Notice of Site Visit” must be posted for 30 consecutive days. Appeal Rights were provided.
