25 | On10/28/2024 at 1:45pm, Licensing Program Analysts (LPAs), Jung and Thompson conducted an unannounced case management-deficiency inspection. Upon arrival, LPAs met with Director, Christina Naffziger. LPAs disclosed the purpose of the inspection and was led on a tour of the facility. There were 63 preschool children, one (1) infant child, and seven (7) staff members present. Hours of operation are Monday through Friday 7:00am to 6:00pm.
During the facility tour, LPAs observed one (1) child who looked like an infant in one of the preschool nap rooms. Upon file review, LPAs were able to confirm that the child was under 24 months old.
Based on the information obtained and interviews conducted, one Type A citation for Limitations of Capacity is being cited on the LIC809D.
LPAs Jung and Thompson informed Director, Christina Naffziger that this report dated 10/28/2024 documents one (1) Type A citation. Type A citation shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.
Also, LPAs Jung and Thompson informed the director to provide a copy of this licensing report dated 10/28/2024 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
